Comments:
An unannounced monitoring visit was conducted from 10:00-12:00. Fifty seven children were present in 4 classrooms with direct care staff. All children were watching a musical performance during the visit, transitioning to hand washing and preparation for lunch. Lunch served today was cheese pizza, salad, apple slices and milk. Reviewed three staff records, emergency medication, a sample of children's records, emergency preparation for drills, training for staff and physical space. . Standard #: 22VAC40-185-270-A
Description: Based on observation, areas of the center are not being maintained in a clean,safe or operable manner. Evidence: 1. Paint is peeling on the red train tracks piece of climbing equipment. 2. The yellow cot wall is peeling paint in the two's classroom. 3. A metal bracket along the wooden rail leading into the older 4's classroom is rusted and broken.

Plan of Correction: A maintenance order to repair paint and bracket was placed during the inspection.

Standard #: 22VAC40-185-290-3
Description: Based on observation, an outlet was missing a protective cover. Evidence: An electrical outlet in the younger 4's classroom by the computer was missing a protective cover.

Plan of Correction: Will be replaced as soon as possible.
