Today an unannounced renewal study was conducted beginning at 2:00pm and ended at 3:35pm. There were 23 children present today with 2 staff. Children were observed arriving from school, greeting the staff, washing hands and eating snack. Children?s records, medications and emergency drills were reviewed. Standard #: 22VAC40-185-140-A Description: Based on record review, children's records did not contain a physical exam. Evidence: Child #5's file did not contain documentation of a physical exam. Plan of Correction: I'll let me boss know and we'll continue to discuss this at our meetings.
Standard #: 22VAC40-185-70-A Description: Based on record review and interview, staff records kept on site did not contain all required information. Evidence: Staff #1 file did not contain an emergency contact to be reached in case of an emergency. The site leader stated that the form had been filled out but it cannot be located. Plan of Correction: This was corrected during the inspection.
Standard #: 22VAC40-185-510-E Description: Based on observation, medication was not labeled with each child's name. Evidence: 1 out of 3 medications did not have the child's name written on it to identify it's owner. Plan of Correction: This was corrected during the inspection.
Standard #: 22VAC40-185-550-D Description: Based on a review of records and interview, the center did not document the implementation of monthly practice of evacuation drills. Evidence: There is no fire drill documented for the month of November. The site leader has stated that she know's a drill was done but it did not get documented on the monthly log. Plan of Correction: I'll start doing this as soon as we walk in the door.
