63.2 Child Abuse & Neglect

Comments:
An unannounced mandated Renewal inspection during the Conditional Licensure period was conducted today from 10:00am to 3:00 pm. There were 15 children present with 3 staff members. Children were observed making the letter T out of Cheerios and pipe cleaners, practicing their letters, having fee play, reading books, washing hands for lunch, eating lunch (fish sticks, broccoli, mashed potatoes and fruit) and napping. Reviewed 3 children's files, 3 staff files, attendance, menu, emergency evacuation log and supplies, outdoor playground, and physical plant. Violations:
Standard #: 22VAC40-185-40-E
Description: Based on observation, center is not ensuring the terms of the current license issued by the department.
Evidence:
Two children, age 2 years, were present during this inspection. The center license is for children ages 2 1/2 yrs and above.

Plan of Correction: We will send out a notice to parents affected.

Standard #: 22VAC40-185-330-B
Description: Based on observation, resilient material does not comply with minimum safety standards for playgrounds.
Evidence:
The resilient material on the preschool playground does not meet safety standards for playgrounds. The mulch has pieces of wood that are 5X7 inches or larger pieces of wood/bark which is a hazard.

Plan of Correction: We will remove all large pieces of mulch/bark.

Standard #: 22VAC40-185-430-B
Description: Based on observation, the center does not have an adequate supply of supplies and equipment for each age group of texture materials, construction materials, music and sound materials, books, social living equipment, and manipulative equipment. Evidence: Rooms do not have an adequate amount of materials for number of children licensed.

Plan of Correction: We will be purchasing more equipment and supplies
