Comments:
An unannounced monitoring inspection was conducted on February 22, 2019. There were 13 children present with two staff members. An additional volunteer was on site for support purposes. The posted menu indicated that for lunch, the children would be served chicken teriyaki, green beans, diced peaches, bread and milk. Observations included the physical plant, nutrition, supervision, staff/child ratios, health and safety, and the children's records. The children were observed doing music and movement, signing, exercising and playing in centers. Five children's records were reviewed. Two medications were reviewed. The outdoor playground was not inspected due to inclement weather.

Standard #: 22VAC40-185-410-1
Description: Based on observation and inspection of the facility, the licensee did not ensure that the forbidden action of restricting movement through binding. Evidence: During the inspection conducted on February 22, 2019, the following was observed: 1. A volunteer was observed sitting on the floor holding a child in her arms and not allowing the child to free himself. The child said to the volunteer "let me go", the volunteer replied "I'm not going to get off until you stop acting up". The child said "get off of me" and the volunteer continued to hold the child in her arms on the floor. The child was squirming in attempts to release himself and said "I'm sorry". The volunteer said "Are you really sorry, why are you being so bad?" 2. The volunteer held the child from approximately 10:41am-10:45am. The volunteer released the child and gave the child a hug.

Plan of Correction: We will have a staff meeting together and explain what Head Start and what licensing allows. We will work on that for all children.
