Technical Assistance:

The licensing inspector conducted an unannounced monitoring inspection Friday, February 22, 2019 from 9:20am to 12:50pm. The children were observed listening to a story and doing exercises. They were also observed singing songs in group time. The infants were observed playing on the floor with their teacher and being fed. Due to rainy weather the playground and the facility?s mini bus and van were not inspected today. Five children?s records and five employee records were reviewed during this inspection. Medication is administered, however the center did not have any today. A sample of five injury reports were reviewed at this time. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 01/22/2019 Last shelter-in-place drill: 09/13/2019 Last fire inspection: 01/29/2019 Last health inspection: 02/01/2019 Total enrollment today: 40 Today, the following child to staff ratios were observed: Infants-3:1 Toddlers ?3:1 Two's-10:2 Preschool ?16:2 Pre-K-8:1 School Age ? not present The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-160-C
Description: Based on a review of staff records on 02/22/2019, the center did not ensure that staff members obtain and submit the results of a follow-up tuberculosis (tb )screening at least every two years. EVIDENCE: The most recent tb screening/test in the record of Staff #2 is dated 02/03/2017.

Plan of Correction: Will have staff get tb screening/test done by 02/27/2019.

Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records on 02/22/2019, the center did not ensure that four of five children's records reviewed did not contain the required information. EVIDENCE: 1. The records of Child #2, Child #4 and Child #5 did not contain the name of any additional programs or schools that the child is concurrently attending and the grade or class level. 2. The records of Child #2 and Child #5 did not contain the name of any previous child day care and schools attended by the child. 3. The most recent update of information in the record of Child #3, who started at the center on 01/07/2016, is dated 01/07/2016.

Plan of Correction: Will get update by 2/22/2019. Will make a copy of front page for parents to complete by 2/25/2019.

Standard #: 22VAC40-185-280-B
Description: Based on observation on 02/22/2019, the center did not ensure that all hazardous substances such as cleaning materials were kept in a locked place. EVIDENCE: 1. In an unlocked office across from the kitchen, disinfectant spray was observed in an unlocked cabinet. 2. Containers of cleaning supplies were observed in an unlocked room located off the hallway in the two's room. The door to the room was open and the gate in the hallway used to prevent access was unlatched.

Plan of Correction: Corrected on site. Items locked up. Before children arrive in the classroom will make sure gate is latched and door is locked.

Standard #: 22VAC40-185-350-C
Description: Based on observation on 02/22/2019, the center did not ensure that in an ongoing mixed age group, the staff-to-children ratio applicable to the youngest child in the group shall apply to the entire group. EVIDENCE: The staff to child ratio for the morning drop off room was four staff to 33 children. The youngest child in this group was 12 months old.

Plan of Correction: Will make sure that there is enough staff to cover ratio. Had two staff call out this morning.
