63.2(17) License & Registration Procedures

Comments:
An unannounced monitoring inspection was conducted on March 1, 2019 from approximately 9:24am-1:35pm. Upon arrival to the facility, there were 43 children with eight staff members. Additional staff were on site for administrative and support purposes. Children were observed doing music and movement, playing with manipulatives, and during storytime. Infants were observed crawling, during tummy time and resting. Licensing Inspector reviewed supervision, activities, equipment, indoor areas, diapering and handwashing procedures, medication, children and staff records, emergency evacuation and shelter-in-place drill. Five children's records and Five staff records were reviewed. Two medications were reviewed. The outdoor playground was not inspected due to inclement weather.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, in two of five children's records reviewed, the licensee did not ensure that each child's record was maintained with all required information. Evidence: During the inspection conducted on March 1, 2019, there was no documentation of the work phone number for one parent in the records for Child #4 and Child #5.

Plan of Correction: Work phone numbers will be added today.

Standard #: 22VAC40-185-70-A
Description: Based on record review, in one of five staff records reviewed, the licensee did not ensure that each staff person's record contained all required information. Evidence: During the inspection conducted on March 1, 2019, there was no documentation that Staff #1 completed orientation training.

Plan of Correction: We are in the process of a new hire for her and that will be done next week.

Standard #: 22VAC40-185-320-B
Description: Based on observation and inspection of the facility, the licensee did not ensure that the water temperature does not exceed 120 degrees. Evidence: During the inspection conducted on March 1, 2019, in Chipmunks classroom, the water temperature of the sink in the restroom and the sink next to the water fountain measured at 121.5F degrees.

Plan of Correction: Assistant Director will notify Director and we will have someone come out and check our water heater.

Standard #: 22VAC40-185-340-D
Description: Based on observation and inspection of the facility, the licensee did not ensure that in each grouping of children at least one staff member who meets the qualifications of a program leader or program director shall be regularly present. Evidence: During the inspection conducted on March 1, 2019, Staff #5 who was observed working alone with a group of seven children in the RainForest classroom and was not program director or program leader qualified.

Plan of Correction: We will change the staff member's status to an assistant until she gains the experience. We do have a lead qualified staff for that classroom.

Standard #: 22VAC40-185-350-E-1
Description: Based on observation and inspection of the facility, the licensee did not ensure that ratios were maintained for children from birth to the ages of 16 months: one staff member for every four children. Evidence: During the inspection conducted on March 1, 2019, there were five children with one staff member in the Woodchucks classroom. The youngest child in the group was 15 months.

Plan of Correction: The ratio was corrected during the inspection. Assistant Director will request permission from the child's parent along with the Director and Teacher's assessment to assign the child to the classroom.

Standard #: 22VAC40-185-500-B
Description: Based on observation and inspection of the facility, the licensee did not ensure that the diapering surface shall be used only for diapering or cleaning children. Evidence: During the inspection conducted on March 1, 2019, in the Oaks I classroom, wipes, a container of pull-ups and gloves and diapers and pullups were observed stored on the diaper changing surface.

Plan of Correction: All items will be removed.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on record review, in one of five staff records reviewed, the licensee did not ensure that staff submit to fingerprinting and obtain results prior to employment. Evidence: During the inspection conducted on March 1, 2019, Staff #1 observed working in the facility on the date of the inspection, started working at the facility on February 28, 2019 and the facility had not obtained fingerprint results.

Plan of Correction: We will obtain fingerprint results for Staff #1.
