Comments:
A Subsidy Health and Safety Inspection was conducted on March 1, 2019 from approximately 9:24am-1:35pm to review supplemental health and safety requirements. Upon arrival to the facility, there were 43 children with eight staff members. Additional staff were on site for administrative and support purposes. Children were observed doing music and movement, playing with manipulatives, and during storytime. Infants were observed crawling, during tummy time and resting. Licensing Inspector reviewed supervision, activities, equipment, indoor areas, diapering and handwashing procedures, medication, children and staff records, emergency evacuation and shelter-in-place drill. Five children's records and Five staff records were reviewed. Two medications were reviewed. The outdoor playground was not inspected due to inclement weather. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: 22VAC40-665-580-B
Description: Based on record review, in two of five staff records reviewed, the vendor did not ensure that staff complete Virginia Preservice Training for Child Care Staff within the first 90 days of employment. Evidence: Staff #2 (date of hire 11/7/18) and Staff #5 (date of hire 10/26/18) had not completed Virginia Preservice Training for Child Care Staff; both staff members have been employed for more than 90 days.

Plan of Correction: We are in the process of eliminating Subsidy clients so we are not requiring new staff to do the Preservice Training.

Standard #: 22VAC40-665-580-E-1
Description: Based on record review, in two of five staff records reviewed, the vendor did not ensure that staff obtain current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care within 90 days of employment. The training shall include an in-person competency demonstration. Evidence: Staff #2 (date of hire 11/7/18) and Staff #5 (date of hire 10/26/18) had not completed an in-person competency demonstration for CPR training.

Plan of Correction: We will have an opener and closer with the in-person competency demonstration training.

Standard #: 22VAC40-665-650-E
Description: Based on observation and inspection of the facility, the vendor did not develop and implement a written policy and procedure that describes how the vendor will ensure that each group of children receives care by consistent staff or team of staff members. Evidence: The center had not developed and implemented a consistent care policy.

Plan of Correction: A consistent care policy will not be created at this time due to part time enrollment.

Standard #: 22VAC40-665-770-B-5
Description: Based on observation and inspection of the facility, the vendor did not ensure that the emergency preparedness plan included continuity of operations procedures to ensure that essential functions are maintained during an emergency. Evidence: The center had not updated the emergency preparedness plan with continuity of operations procedures to ensure that essential functions are maintained during an emergency.

Plan of Correction: We will create a procedure on continuity of our daily operations.
