Technical Assistance:
Technical assistance was provided regarding the out of state background checks.

An unannounced monitoring inspection was conducted today from 9:00am-2:00pm, coinciding with a self report incident. There were 73 children supervised by 10 direct care staff. The physical plant, programing, children's evaluations, staff records, children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Staff was interviewed, along with review of statements and video regarding the self report. Behavior guidance that was not constructive in nature, age and stage was observed. Children were observed participating in group play, reading books, playing outside, during circle time and having lunch. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Description: Based on review of staff records, not all staff have documentation of a negative tuberculosis (TB) screening within 21 days of employment. Evidence: Staff #3, start date 7/18, does not have documentation of a negative TB screening.

Plan of Correction: She has one, the dr. will fax it over.

Description: Based on review of staff records, not all staff have documentation of an updated negative TB screening every 2 years. Evidence: Staff #4 has documentation of a negative TB screening that expired 6/16/18. Staff #5 has documentation of a negative TB screening that expired 1/18/19. Staff #6 has documentation of a negative TB screening that expired 1/13/19

Description: Based on review of children's records, not all records are complete. Evidence: Child #1, 2, & 3 do not have documentation of an annual parental update of their emergency contact information.

Plan of Correction: I will have the parents review the forms and make any changes.

Description: Based on observation, not all areas of the center are maintained in a clean, safe operable condition. Evidence: In the AM Preschool classroom, an air vent that is taped to the wall has peeling tape.

Plan of Correction: Corrected during inspection.

Description: Based on observation, hazardous substances were accessible to children in an unlocked cabinet. Evidence: In the Intermediates classroom, rubbing alcohol was located in an unlocked cabinet, making it accessible to children.

Plan of Correction: Corrected during inspection.

Description: Based on observation, not all electrical outlets have protective covers. Evidence: Power strips in the Pre k and Intermediates classroom have outlets that do not have protective covers.

Plan of Correction: I will put protective covers on all outlets.

Description: Based on interviews with staff, review of video footage and statement from witness, staff did not provide behavioral guidance that was constructive in nature, age and stage appropriate. Evidence: Staff #1was observed by staff #2 and a witness speaking very loudly to child #1, she then became upset with child #1 and pulled him by his arms towards her to sit in the chair. Child #1 did not sit in the chair, staff #1 proceeded to kick the chair and walk away. Child #1 was then comforted by staff #2.

Plan of Correction: Staff #1 is no longer employed.

Standard #: 22VAC40-185-440-B
Description: Based on observation, cots are not identified for use by a specific child. Evidence: Cots in the Pre K 2, Intermediates, Beginners and Toddler classrooms are not labeled for use by an individual child.

Plan of Correction: We will label each cot.

Standard #: 22VAC40-185-550-M
Description: Based on review of incident/accident reports the center does not maintain a record of notification to parents. Evidence: Incident/accident reports were not completed with notification to parents to include date and time.

Plan of Correction: Going forward we will include the date and time parents were notified.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on review of staff records, not all out of state central criminal records have been completed for staff that have lived outside the state of Virginia within the past 5 years. Evidence: Staff # 4 has lived in 2 states outside the state of Virginia within the past 5 years and does not have documentation of central criminal records search for either state.

Plan of Correction: I have printed off the forms and will mail them off this week. When the results are received I will email them to you.
