Comments:
The Licensing Inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing office on 3/1/19. Observations of classrooms and interviews were conducted with staff relating to the allegations of ratios. The Licensing Inspector arrived at 7:58 am to find two of the three classrooms out of ratio. All three classroom were out of ratio by 8:26 until 9:00 am. Information gathered during this investigation supports the allegations, so the complaint is determined to be valid. Description: Based on observation and staff interview, one staff for every 4 children is not maintained in the infant room. Evidence: The Licensing Inspector (LI) arrived at the center at 7:58 am. At 8:19 am there were 2 staff with 9 infants. At 8:26 am the 10th infant arrived. The class remained out of ratio with 2 staff and 10 infants until a third staff arrived at 9:00 am. At 7:58am there was 1 staff with 9 infants, the youngest being 13 months and toddlers in the crawlers room. At 8:15 am there were 2 staff with 11 infants and toddlers, the youngest being 13 months. At 8:30 am there were 2 staff with 14 infant and toddlers, the youngest being 13 months. At 8:55 am there were 3 staff with 17 infants and toddlers, the youngest being 13 months. The class remained out of ratio with 3 staff and 17 infants and toddlers until two other staff arrived at 9:00 am.

Plan of Correction: We will review the schedules to see if we can have staff come in earlier. The owner is advertising and interviewing for additional positions.

Evidence: The Licensing Inspector (LI) arrived at the center at 7:58 am. At 8:00 am there was 1 staff with 13 children the youngest being 2 years old. At 8:15 am there was 1 staff with 15 children, the youngest being 2 years old. At 8:25 am there was 1 staff with 18 children, the youngest being 2 years old. The class remained out of ratio with 1 staff and 18 children until the second staff arrived at 8:30 am. Once the children split into their classrooms at 8:35, there was 1 staff with 11 children, the youngest being 2 years old. At 8:45 there was 1 staff with 13 children, the youngest being 2 years old. The class stayed out of ratio with 1 staff and 13 children until the second staff arrived at 8:55.

Plan of Correction: We will review the schedules to see if we can have staff come in earlier. The owner is advertising and interviewing for additional positions.
