Comments:
The inspector conducted an unannounced focused inspection on 03/01/2019 from 8:57am to 12:58pm. This inspection focused on compliance with the standards that could not be determined during the initial inspection due to the center not being in operation at the time. The children were observed playing in centers, painting, singing and eating morning snack and lunch. A formal observation was completed with the preschool students. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children when appropriate. Five children?s records and four staff records were reviewed during this inspection. The center's first aid kit and emergency supplies were inspected. Violations:
Standard #: 22VAC40-185-70-A
Description: Based on record review, the center did not ensure that one of four staff records had documentation of two or more references as to character and reputation as well as competency were checked before employment. Evidence: The record of staff #2 (start date: 1/22/19) did not have documentation of references.

Plan of Correction: Will get references checked today.

Standard #: 22VAC40-185-240-A
Description: Based on record review, the center did not ensure that three of four staff records had documentation of the required orientation training by the end of the first day of employment. Evidence: The record of staff #1 (start date: 1/22/19), staff #2 (start date: 1/22/19) and staff #4 (start date: 1/07/19) did not have documentation of the required orientation training.

Plan of Correction: Will be completed on 3/4/19.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure that hazardous substances such as cleaning materials, were kept in a locked place using a safe locking method that prevents access by children.
Evidence:
Classroom #1 had a container of sanitizing wipes on top of the children's cubbies. The hallway closet was unlocked and contained several cleaning products.

Plan of Correction: Corrected during the inspection.

Standard #: 22VAC40-185-290-3
Description: Based on observation, the center did not ensure that electrical outlets shall have protective covers that are of a size that cannot be swallowed by children. Evidence: Classroom #1 and classroom #2 both had one uncovered outlet within the reach of children.

Plan of Correction: Corrected during the inspection.

Standard #: 22VAC40-185-340-D
Description: Based on observation and record review, the center did not ensure that in each grouping of children at least one staff member met the qualifications of a program leader. Evidence: The inspector observed staff #1 and staff #2 supervising a group of eight children during the inspection. The center's representative informed the inspector that staff #1 is a program leader, staff #2 is an aid and that staff #2 is usually left alone with children. The record of staff #2 revealed that the staff member did not have all of the required documentation to meet program leader requirements.

Plan of Correction: Staff #1 will obtain all training & documents on 3/4/19.
