63.2(17) License & Registration Procedures
An unannounced renewal inspection was conducted on March 8, 2019 to determine the facilities compliance with licensing standards. The inspector arrived at 9:00 am and concluded the inspection at 11:30 am. There were fifteen children in care with two staff. Upon the inspector's arrival children were working in centers. Children constructed with blocks, painted at the easel, had computer time, played with puppets and play-dough. Staff assisted with an art project of painting butterfly wings. Later children gathered for a circle time where they danced to music and did stretching exercises with staff. Staff interacted with the children in a positive manner. Staff cleaned and sanitized tables. Children had bathroom breaks and hand washing was observed in preparation for lunch. Today's lunch consisted of chicken patty on a bun, broccoli with cheese sauce, carrots with dip, fruit and milk. All areas of the center were inspected to include the main hallway, cafeteria, bathroom, classroom and outdoor playground area. The facility had all required information posted to include the license, last inspection notice, emergency numbers, menu, evacuation map/procedures, daily schedule and various parent information. The classroom had age and stage appropriate materials. The first aid kit and non-medical supplies were inspected and found complete. The inspector interacted with staff and children in care. Five children's records and three staff records were reviewed. Five records for board officers were also reviewed. Staff have current CPR, first aid, daily health observation training and MAT. Medications and authorizations were reviewed. An evacuation drill was conducted on 02/05/2019 The two required shelter-in-place drill for the 2018-2019 year were completed on 09/25/2018 and 10/18/2018. Fire Inspection: 08/30/2018 Health Inspection: 12/19/2018 Violations cited during the licensing period were checked for corrections. There were no repeat violations. Standard #: 22VAC40-185-60-A Description: Based on review of five children's records, the facility did not obtain all required information for one child's record. Evidence: The record of child #1, enrolled 09/11/2018, did not have the addresses for the two emergency contacts listed. Plan of Correction: Per the director: We will contact the parent and get the missing information for the record.
Standard #: 22VAC40-191-40-D-1-A Description: Based on review of five board officer records, the facility did not obtain a sworn statement for a newly designated agent within the required time frame. Evidence: Board officer #3 was appointed on 10/25/2018. The documented sworn statement was dated 01/18/2019. Sworn statements for newly appointed board officers are required to be obtained within the first 30 days of being appointed. Plan of Correction: Per the administrator: We will use the same systematic systems we use to process background checks for our employees. Background checks will be obtained before officers are appointed to the board to ensure they are completed within the time requirements.
Standard #: 63.2(17)-1721.1-B-2 Description: Based on review of five board officer records, the facility did not obtain a fingerprint based national background check within the required time frame. Evidence: 1. The fingerprint based national background check for board officer #2 was dated 01/28/2019. Board officers that were appointed prior to 01/21/2018 were required to obtain the fingerprint based national background check by 09/30/2018. 2. The fingerprint based national background check for board officer #3 who was appointed on 10/25/2018) was dated 01/30/2019. Fingerprint based national background checks are required to be obtained within the first 30 days of being appointed. Plan of Correction: Per the administrator: We will use the same systematic systems we use to process background checks for our employees. Background checks will be obtained before officers are appointed to the board to ensure they are completed within the time requirements.
