The licensing inspector conducted an unannounced monitoring inspection on 02/26/2019 from 9:30am to 2:25pm. The children were observed participating in story time and playing. A formal observation was completed with the infant and toddler classrooms. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today as well as the mini buses. Five children?s records and five employee records were reviewed during this inspection. Medication is being administered and was reviewed. The center?s injury prevention plan was reviewed and was updated 01/2019. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 02/6/2019 Last shelter-in-place drill: 09/17/2018 Last fire inspection: 04/11/2018 Last health inspection: 01/08/2019 Today, the following child to staff ratios were observed: Infants - 7:2 Toddlers ? 4:2 Twos - 8:1 Preschool ? 16:2 Pre - Kindergarten- 10:2 Transitional K - 9:1 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-70-A Description: Based on record review, the center did not ensure that one of five staff records contained documentation of two or more references as to character and reputation as well as competency before employment. Evidence: The record of staff #4 (start date: 12/3/18) contained references dated 12/4/18. Plan of Correction: We will ensue all references are complete before the start of employment. Center Management will assure this is implemented.
Standard #: 22VAC40-191-60-B Description: Based on record review, the center did not ensure that three of five staff records contained documentation of a sworn statement of affirmation prior to the first day of employment: Evidence: 1. The record of staff #1 (start date: 1/21/2019) did not contain documentation of a sworn statement of affirmation. 2. The record of staff #2 (start date: 1/21/2019) and staff #3 (start date: 1/21/2019) both had documentation of a sworn statement of affirmation dated 1/24/2019. Plan of Correction: We will ensure all new hires have sworn statements complete the day of hire. Staff #1 has been completed. Center Management will complete forms the day of hire.
Standard #: 22VAC40-191-60-C-2 Description: Based on record review, the center did not ensure that one of five staff records contained documentation of central registry results within 30 days of employment. Evidence: The record of staff #3 (start date: 1/21/2019) did not have documentation of central registry results. Plan of Correction: We will resubmit document to central registry. Documentation copied and saved after several attempts to Central Registry. Center Management will complete.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on record review, the center did not ensure that one of five staff records contained documentation of an out of state child abuse and neglect search request within 30 days of employment. Evidence: The record of staff #4 (start date: 12/3/2018) contained documentation confirming that the staff has lived out of the state within the last 5 years and did not contain documentation of an out of state child abuse and neglect search request. Plan of Correction: We will ensure all new hire background checks are complete. Staff #4 - The previous state she lived in doesn't do out of state background checks. Documents are in file.
