A Subsidy Health and Safety inspection was conducted on 02/26/2019 from 9:30am to 2:25pm to review supplemental health and safety requirements. The children were observed participating in story time and playing. A formal observation was completed with the infant and toddler classrooms. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today as well as the mini buses. Five children?s records and five employee records were reviewed during this inspection. Medication is being administered and was reviewed. The center?s injury prevention plan was reviewed and was updated 01/2019. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 02/6/2019 Last shelter-in-place drill: 09/17/2018 Last fire inspection: 04/11/2018 Last health inspection: 01/08/2019 Last lock down drill: 09/27/2018 Today, the following child to staff ratios were observed: Infants - 7:2 Toddlers ? 4:2 Twos - 8:1 Preschool ? 16:2 Pre - Kindergarten- 10:2 Transitional K - 9:1 A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. A violation was found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: 22VAC40-665-540-A Description: Based on record review, the center did not ensure that three of five staff records contained documentation that their tuberculosis screening was submitted at the time of employment and that the documentation was completed within the last 30 days. Evidence: 1. The record of staff #1 (start date: 1/21/2019) and staff #3 (start date: 1/21/2019) both had documentation of a TB screening dated 1/24/2019. TB screenings should be obtained at the time of employment. 2. The record of staff #2 (start date: 1/21/2019) had documentation of a TB screening dated 4/11/2018. TB screenings should be completed within the last 30 days of employment. Plan of Correction: We will ensure all new hires have sworn statements complete the day of hire. Staff #1 has been completed. Center Management will complete forms the day of hire.
