Technical Assistance:
Reviewed the new requirements for subsidy health and safety inspections.

Comments:
Licensing inspector arrived at 11:00 am and departed at 2:30pm to conduct a licensing inspection and a Subsidy Health and Safety inspection. At the time of the tour there were twenty- four children in care with 4 staff . Children were observed freely playing, listening to music, napping and eating lunch. The children had meat sauce with spaghetti, pineapple and corn. The sample size consisted of 6 children records and 6 staff records. Areas of non-compliance was reviewed with the director.

Violations:
Standard #: 22VAC40-665-510-B
Description: Based on record review and staff interview, the vendor did not ensure for each group of children, the vendor shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: During the inspection conducted on March 12, 2019, the attendance document for March 11, 2019 did not have documentation of departure for all children. The attendance document for March 12, 2019 did not have documentation of arrival of all children.

Plan of Correction: Was corrected today. We will do it at point of service.

Standard #: 22VAC40-665-580-F
Description: Based on interview and 1 of 6 records reviewed, the vendor did not ensure staff providing care to children had CPR and First Aid within 90 days of the date of employment or current certifications. Evidence: During the subsidy inspection conducted on March 12, 2019, staff #2( hire date 11/12/2018 ) did not have CPR and First aid training with in 90 days from date of employment.

Plan of Correction: CPR and First aid is being schedule. And she will have class by March 29, 2019.

Description: Based on record review and staff interview, the licensee did not ensure a written policy and procedure was developed that describes how the vendor will ensure that each group of children receives care by consistent staff or team of staff members. Evidence : On March 12, 2019, the center did not develop a written policy and procedure to ensure group of children receives care by consistent staff. Confirmed with staff .

Plan of Correction: The provider will develop a procedure for ensure group of children receives care by consistent staff.

Standard #: 22VAC40-665-770-B-5
Description: Based on record review, the licensee did not ensure there was continuity of operation procedures to ensure essential function are maintained during and emergency. Evidence: On March 12, 2019, the center did not add the procedures for continuity of operations to ensure that essential functions are maintained during an emergency in the center's emergency plan. Confirmed by staff in charge.

Plan of Correction: The provider will develop a procedure for ensure group of children receives care by consistent staff.
