Comments:
An unannounced renewal inspection was completed on March 14, 2019 from approximately 9:30a.m. to 12:15p.m. Fifty-three children were in care during the inspection. Staff and children were observed in five classrooms and outside with adequate staff ratios. There are six classrooms but one was not in use during the inspection. Children and staff were observed engaging in activities to include free play time, center time, snack time, taking spring pictures and outside play. Children?s equipment, learning materials, outdoor and classroom space, rest rooms and first aid and emergency supplies were inspected. Five children?s records, five staff records, one agent of the organization's record, annual fire and health inspections, fire drill and shelter in place logs, daily health observation and medication administration certifications, injury reports, daily attendance, the liability insurance policy, the emergency preparedness plan, the injury prevention plan, medication authorizations and logs and required postings were reviewed. The Director was available for interview and the inspection and was present at the exit interview at which time inspection findings were reviewed. There were two citations for violations of the Standards for Licensed Child Day Centers. See the violation notice on the Department?s public web site for violations of the Standards.

Violations:
Standard #: 22VAC40-185-230-C
Description: Based on observation and interview, the licensee failed to ensure that volunteers who work with children shall be at least 13 years of age. Evidence: 1. A volunteer was observed in a classroom, working directly with children, four and five years old, at a table during center time. 2. In an interview with Administrator #1, it was reported that the volunteer is eleven years old and volunteers at the center approximately once per week.

Plan of Correction: The Director will notify the volunteer's parent that the child is no longer able to volunteer until he turns 13 years old.

Standard #: 22VAC40-185-510-D
Description: Based on a review of medication authorizations, the licensee failed to maintain available medical authorizations for medication during the entire time it is effective. Evidence: 1. A prescription epi-pen was observed for Child #9. A medication authorization was not present for this medication. 2. Administrator #1 acknowledged the center does not have an authorization for this medication.

Plan of Correction: The Director will contact the parents and will obtain an authorization.
