An unannounced renewal inspection was conducted from 11:00 am-2:20pm. 78 children were in care with 17 staff members present. Children were observed engaged in group activities, including Spanish curriculum lessons, playing outside, reading books and art/crafts projects. 5 staff and 5 children records, medications, emergency drill records, and first aid kit were reviewed. The lunch menu included black beans, corn, chicken quesidillas, green beans, fruit and milk. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-270-A Description: Based on observation, areas inside the center were not maintained in a safe and operable condition. Evidence: 1. In the Preschool classroom, peeling paint was found on the blue wall by the sink. 2. In the K-Prep classroom, the baseboard by the glass doors facing the playground was coming away from the wall. Plan of Correction: A work order will be placed for all maintenance work.
Standard #: 22VAC40-185-430-B Description: Based on observation, materials used for children were not age appropriate. Evidence: In the Two's-2 classroom water beads were used by the children. Product details for the water beads indicate that they are not to be used by children under the age of 3 years. Plan of Correction: The water beads will no longer be used by the 2 year old children.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on record review and staff interview, the center did not obtain out of state central registry findings for staff that has resided outside the state of Virginia in the last five years. Evidence: 1. Staff # 1 was hired on 11/27/18. The out of state central registry form for Maryland was mailed for Staff # 1 on 11/29/18. The results were not obtained within 45 days and no follow up was done to inquire about the results. 2. Staff # 2 was hired on 1/7/19. The out of state central registry form for Maryland was mailed for Staff # 2 on 1/17/19. The results were not obtained within 45 days and no follow up was done to inquire about the results. 3. Staff # 3's out of state central registry form for Washington DC (DC) was re-sent on 9/27/18, after no results were obtained in the first attempt. Staff # 4 conducted the first follow up on 10/22/18. An additional follow up was not conducted to inquire about the results. The results have not been obtained. 4. Staff # 5 's out of state central registry for DC for re-sent on 12/3/18, after no results were obtained in the first attempt . Staff # 4 did not conduct a follow up to inquire about the results. The center has not yet obtained the out of state central registry results for Staff # 5. Plan of Correction: The agencies will be called as a follow up. If needed, new forms will be submitted.
