Technical Assistance:
Discussed the mulch on the playground.

Comments:
An unannounced monitoring inspection was conducted from 8:30am-10:50am. At the time of entrance, 8 children were in care with 3 staff members present. Children were listening to the teacher read a book and playing with toys. A selection of staff and children records, the physical space, emergency supplies, evacuation drills, attendance records and the injury prevention plan were reviewed. Interactions between the children and staff were positive. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-70-A
Description: Based on record review, staff records did not include all required information. Evidence: 1. Staff #5's file (date of hire 3/13/2019) had documentation of one reference, but did not have a 2nd reference checked before employment.

Plan of Correction: I am going to ask for a new reference and reach out to that reference. I had called the 2nd reference three times and they had not returned my call due to being in a different time zone and State.

Standard #: 22VAC40-185-340-D
Description: Based on observation and record review, the center did not have at least one staff member in each grouping of children who meets the qualifications of a program leader or program director. Evidence: 1. During the inspection, the Toddler and Two year old children were combined from 8:30am to 9:00am. Two staff were supervising the children and neither Staff #1 or Staff #2 were qualified as a program leader or program director.

Plan of Correction: The Pre-K Lead teacher will not separate from the Toddler Two's class until 9:00am when the director arrives to help in the Toddler and Two's classroom.
