Technical assistance was provided in the following areas of the standards: 22VAC40-191 (Background checks); 22VAC40-185-(3)-210 (Program leader qualifications); 22VAC40-185-(7)-550 (Emergency procedures); 22VAC40-185-(8)-570 (Infant feeding)
An unannounced monitoring inspection was conducted on 3/6/19 from 9:45am - 11:45am. During the inspection there were 22 children ages five months old through four years old in care with seven staff. Children were observed participating in various activities in the classrooms and eating lunch. Records were reviewed for five children and five staff. There was no medication at the center. Emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.
Standard #: 22VAC40-185-70-A Description: Based on a review of five staff records, it was determined that the licensee did not ensure that a record is kept for each staff will all of the required items. Evidence: 1. The record for staff #3 (date of hire 2/19/19) did not include the name, phone number and address for a person to contact in an emergency, and only included one written reference check. 2. Staff #6 reviewed the record for staff #3 and confirmed that it did not contain all of the required items. Plan of Correction: The facility responded: The missing items for staff #3 will be completed and placed in the file.
Standard #: 22VAC40-185-270-A Description: Based on on observation, it was determined that the licensee did not ensure that all areas and equipment of the center, inside and outside, shall be maintained in a safe condition. Evidence: 1. There was a dangling cord from a radio on shelf in the Toddler/Two's classroom that was a hazard to the children. 2. Staff #6 confirmed that the location of the radio and it's cord created an unsafe condition. Plan of Correction: The facility: Corrected during the inspection. Staff will regularly check to ensure that there are no unsafe conditions in the center.
Standard #: 22VAC40-185-340-D Description: Based on observation, a review of five staff records and interviews, it was determined that the licensee did not ensure that in each grouping of children at least one staff member who meets the qualifications of a program leader or program director shall be regularly present. Evidence: 1. Staff #4 was working alone with 6 children in the Pre-k classroom. The record for staff #4 did not contain documentation to demonstrate that she was Program Leader qualified. 2. Staff #6 reviewed the record for staff #4, and confirmed that it did not contain documentation to demonstrate that she was Program Leader qualified. Plan of Correction: The facility responded: Staff #4 will not work alone until completing the required training to become a Program Leader. We will ensure that there is a Program Leader in each grouping of children.
