Comments:
An unannounced monitoring inspection was completed on 03/20/2019 from 11:00 A.M.-4:30 P.M. At the time of the inspection 41 children were in care with twelve staff, one office administrator, and one administrator present. An additional 12 children arrived after school and were in the care of one staff member. Five children records, five staff records, four injury reports, emergency plan, first aid kits, evacuation maps, emergency supplies, playground, and posted required information were reviewed. Children were observed during storytime, handwashing, lunchtime, transition to naptime, behavioral guidance, infant interactions, naptime, snack, and afternoon center play. School age children were observed in an activity with Master Gardener's. They were turning the soil and preparing to have it sent off for testing. The exceptions to compliance are noted on the violation notice with one repeat violation cited. Description: Based on a observation, the center failed to ensure that all hazardous materials were kept in a locked location. Evidence: 1. In the Pre-K Bathroom on a shelve there was a can of Glade Sultry Amber Rhythm Air Freshner. 2. In the Three Year Old Classroom the laundry room door was unlocked and contained three tubs of White Powder Laundry Detergent & Concentrate. 3. The warning labels stated "keep out of reach of children", "hazard", and "caution". 4. Staff verified that the materials were not kept in a locked location.

Plan of Correction: The administrator will meet with staff and reinforce the standard of keeping all hazardous materials in a locked location, and start doing spot checks to ensure all hazardous materials are kept locked.
