Comments:
Today an unannounced Renewal Inspection was conducted between the hours of 11:20am and 2:45pm. There were 2 classrooms observed with a total of 44 children with 10 staff within the supervision guidelines. The children were observed playing outside, coming inside and washing hands, eating lunch (ravioli, peas and strawberries) and napping. A complete inspection of the physical plant, children and staff records, fire drill log, medication and emergency procedures were observed during this inspection. Violations:
Standard #: 22VAC40-185-510-C
Description: Based on record review, expired medication was not returned to the parents. Evidence: Child #7's medication expired 7/16/2017. The medication was observed on site during today's inspection.

Plan of Correction: We'll return the medication to the parents tonight. We'll start doing monthly audit meetings to review medications and authorizations.

Standard #: 22VAC40-185-510-N
Description: Based on record review and observation, the center has not returned medication with expired authorization to administer to a child's parents. Evidence: The authorization to administer medication to Child #6 expired on 2/28/2019. Child #6 was observed receiving the medication during today's inspection.

Plan of Correction: We're expecting a new form from these parents soon. We'll have the parents fill out the short-term form until the long-term form can be completed by the doctor. We'll be doing monthly audit meetings to review medications and authorizations.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on record review and interview, the center has not obtained an out-of-state central registry (CPS) background check for one employee that has lived outside of the state of Virginia within the last five years. Evidence: Staff #1's sworn disclosure statement confirms that they have lived outside of the state of Virginia within the last five years. There was no documentation in the employee's file of the out-of-state central registry check. Center management has confirmed that an out-of-state check has not been completed.

Plan of Correction: This staff member was given the form during today's inspection. It will be filled out and notarized today and in the mail by Friday.
