Today we discussed the outdoor play area and tripping hazards. The root system on the large tree located on the playground has become increasingly exposed as the ground is worn away. Please consider methods for replenishing soil or ground cover in these areas sufficiently to minimize the likelihood that children could catch their foot on a root.
An unannounced monitoring inspection was conducted from. to 11:40 a.m. to 2:40 p.m. At the time of entrance the following adult to child ratios were observed: Classroom D: 17 children, ages 3 to 4 years old, with 2 staff Classroom A: 15 children, ages 3 to 4 years old, with 2 staff Classroom B: 17 children, ages 3 to 4 years old, with 2 staff Classroom C: 18 children, ages 3 to 4 years old, with 2 staff The records sample size consisted of 7 children?s records. Staff records are housed off site and will be reviewed at a separate inspection. Children were observed eating lunch which consisted of Sausage and cheese pizza, broccoli, mixed fruit and milk. Hand washing, table sanitizing procedures, restroom procedures, rest period and afternoon snack were also observed. Medication administration was not observed. However, 13 medications kept on site for emergency use were reviewed with regard to storage, written authorizations and required documentation. Thank you for your time during today's inspection. Standard #: 22VAC40-185-270-A Description: Based on observation, the program did not ensure that all areas and equipment of the center, inside and out, are maintained in clean, safe, operable condition. Evidence: 1. Two of the posts (blue) on the outdoor slide structure have peeling paint which is easily chipped away by brushing fingers across the surface. The posts have been repainted in areas since the last inspection. However, even the freshly painted areas are peeling and splintering - and may require thorough sanding to prepare the surface before repainting. 2. The shed located on the outdoor playground has areas of broken / chipped pieces of wood and areas where the wood is splintering. 3. There is the potential for children to put pieces of chipped paint in their mouth from the peeled paint on both of the blue posts on the play structure or to put pieces of splintering wood in their mouth or to get a splinter in their skin from the raw rough wood on the shed. Plan of Correction: The contact staff (Staff #1) stated that she will submit a work request today to have these items repaired.
