Comments:
An unannounced monitoring inspection was completed on March 18, 2019 from approximately 11:30a.m. to 1:00p.m. Twelve children, six to fourteen months old, were in care during the inspection. Staff and children were observed in one classroom space with adequate staff ratios. Children and staff were observed engaging in infant care and activities to include tummy time, feeding, returning from a stroller walk, play, sleeping and diapering. A volunteer was also present. Children?s equipment, learning materials, and the outdoor and indoor space were inspected. Five children?s records, two new staff records, infant reports, daily attendance and required postings were reviewed. There were no injury reports. The Program Lead and Director were available for interview and the inspection and the Director was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. There were three citations for violations of the Standards for Licensed Child Day Centers. See the violation notice on the Department?s public web site for violations of the Standards.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on a review of children?s records, the licensee failed to obtain an immunization record for a child before the child attended the center for one of five children. Evidence: 1. Child #1's record, enrolled on March 1, 2019, lacked documentation of immunizations. 2. Administrator #1 acknowledged an immunization record was not obtained.

Plan of Correction: The Director will obtain the record from the parent.

Standard #: 22VAC40-185-60-A
Description: Based on a review of children?s records, the licensee failed to maintain all the elements of a child?s record for one of five children. Evidence: 1. Child #3's record, enrolled February 14, 2019 and Child #4's record, enrolled March 2019 lacked complete emergency contact addresses. 2. Administrator #1 acknowledged this information was not documented.

Plan of Correction: The Program Lead will obtain this information from the parents today.

Standard #: 22VAC40-185-90--A
Description: Based on a review of children's records, the licensee failed to maintain a written agreement between the parent and the center by the first day of the child's attendance. Evidence: 1. Child #2's record, enrolled on February 18, 2019, lacked a written agreement between the parent and the child day center. 2. Administrator #1 acknowledged the agreement was not between the child day center and the parent but was an agreement used by the previous child day center owner.

Plan of Correction: The Program Lead will have the parent complete the correct forms.
