Comments:
An unannounced monitoring inspection was completed on March 19, 2019 from approximately 1:20p.m. to 3:45p.m. One hundred and eight children were in care during the inspection. Staff and children, ages infant to five to years old, were observed in eight classrooms with adequate staff ratios. There is an additional school age classroom but those children had not arrived. Children and staff were observed engaging in activities to include nap time, waking up from nap time, hand washing, snack time and outside play. Children?s equipment, learning materials, outdoor and classroom space, rest rooms and the first aid and emergency supplies were inspected. Eleven children?s records, ten staff records, annual fire and health inspections, fire drill, shelter in place and lock down drill logs, daily health observation and medication administration certifications, injury reports, daily attendance, the emergency preparedness plan, medication authorizations and logs and required postings were reviewed. The Director was available for interview and the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. There were two citations for violations of the Standards. See the violation notice on the Department?s public web site for violations of the Standards.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children?s records, the licensee failed to maintain all the elements of a child?s record for two of eleven children. Evidence: 1. Child #6's record, enrolled on July 25, 2018, and Child #11's record, enrolled on September 4, 2018, lacked the name of a parent's place of employment. 2. Administrator #2 acknowledged this information was not documented.

Plan of Correction: The Director will obtain the missing information from the parents.

Standard #: 22VAC40-185-270-A
Description: Based on observation of outdoor space, the licensee failed to ensure that areas and equipment of the center are maintained in a clean, safe and operable condition. Evidence: 1. Protruding nails, a loose board and an area of broken chain link fence were observed on the play gound, accessible to children and creating hazards. 2. Administrator #1 acknowledged these hazardous items on the playground.

Plan of Correction: These items were correced on site by a staff member.
