63.2(17) License & Registration Procedures
An unannounced monitoring inspection was conducted on March 27, 2019 from approximately 3:40pm-. Upon arrival to the facility, during the aftercare portion of the program, there were two staff present with 27 school age children. The children were observed coloring, playing with blocks, doing homework, playing freely and having snack which included apple slices with caramel and apple juice. Five children's records were reviewed on this date. Staff fingerprints were observed on this date. Five medications were reviewed. Licensing Inspector reviewed supervision, activities, equipment, nutrition, children's records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports.
Standard #: 22VAC40-185-510-C Description: Based on review and inspection of the facility, the center did not follow their outdated medication policy. Evidence: During the inspection conducted on March 27, 2019, in the center's medication box, there was a medication that expired in February 2019. The center's policy states the following: All medications must be checked for expiration monthly. The center is to notify parents that medications are soon expiring 14 days prior to expiration. The center is to complete a Medication Return slip and return the medication to the parent. If the medication is not returned to the parent, the center is to complete a medication disposal slip prior to disposal. It further states that outdated medications should not be left at site locations. Plan of Correction: Site Director will submit a medication return form and inquire if the parent would like to continue to have the medication on site.
Standard #: 22VAC40-185-510-N Description: Based on observation and inspection of the facility, the licensee did not ensure that when an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center. Evidence: During the inspection conducted on March 27, 2019, a medication authorization form was effective until 2/2/19 and the medication had not been returned to the parent or discarded of by the center, as the medication was observed in the center's medication box. Plan of Correction: Site Director will talk to the parent about filling out a current medication authorization form.
Standard #: 22VAC40-185-550-D Description: Based on review, the center did not implement a monthly practice evacuation drill. Evidence: During the inspection conducted on March 27, 2019, there was no documentation that a practice evacuation drill was conducted in October 2018, December 2018 and January 2019. Plan of Correction: Site Director will make sure all drills are scheduled monthly and documented.
Standard #: 22VAC40-185-560-F Description: Based on observation and inspection of the facility, the licensee did not ensure that when centers choose to provide meals or snacks, the centers shall follow the most recent, age-appropriate nutritional requirements. Evidence: During the inspection conducted on March 27, 2019, the children were observed having apple slices with caramel sauce and apple juice. The USDA requires children to have two different snack components. Plan of Correction: Supervisor will diversify the snack calendar.
