Technical Assistance:
We discussed the requirements for children's records. The risk assessments for violations were provided. Also, we reviewed behavior management during meals and making sure food is not removed before children have finished eating. We talked about conducting an annual update to the injury prevention procedures.

Comments:
Thank you for your assistance during the unannounced renewal inspections conducted on March 26, 2019 from 3:09 PM to 4:10 PM and on March 28, 2019 from 9:48 AM to 2:05 PM. Today, there were fourteen children, ages three through five years, with three to four staff. I viewed program activities, staffing, supervision, interactions with children, equipment, posted information, menu/nutrition, injury reports, emergency supplies and drill records, agency inspections, six staff records, staff qualifications, staff training, medication, five records for children, indoor and outdoor areas. The outdoor equipment remains off limits due to the need to replenish the mulch in use zones. The staff were observed providing attentive supervision and responding to individual needs for care and guidance. A variety of learning experiences were available at classroom centers. The children have enjoyed activities using materials for the learning theme of "reduce, reuse and recycle". Songs and fingerplays led by staff are used during transitions.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on a review of five records for children, and an interview with administrative staff, the center failed to obtain a physical examination report within the first thirty days of initial attendance by one child. Evidence: Child 2 started attending on November 14, 2018 and the physical examination FAX date of receipt was not until January 8, 2019.

Plan of Correction: The family advocate or assigned administrative staff will make sure the physical examination record for new children is obtained within thirty days if it is not submitted by the parent prior to enrollment. The family advocate will use a system for counting down the days before the record is due.

Standard #: 22VAC40-185-60-A
Description: Based on a review of five records for children, and an interview with administrative staff, the center failed to obtain all required written information for children at the time of enrollment for two children. Evidence: The workplace for one parent of child 3 and the workplace of one parent of child 5 were not written in the records. Administrative staff indicated that a new form replaced the previous registration form beginning in 2019 and only had a place for writing the work phone number.

Plan of Correction: The family advocate immediately started seeking and adding missing workplace information to the children's records. A model registration form will be added for use so that all required information is obtained for children's records.
