Technical Assistance:
Consultation provided on standard 560K (sanitizing tables), 570A (feeding children under 12 months), 500.B.4 (diaper changing procedures.) Also provided consultation regarding the infant room and communication with other staff about emergencies and other immediate needs.

Comments:
An unannounced complaint inspection was conducted from 10:00 am - 12:35 pm with the local Department of Social Services regarding Supervision of Children: care, protection and guidance, and sight and sound supervision. Interviews were conducted, observations were made, and documentation was reviewed. The portion of the complaint regarding sight and sound supervision was determined to be valid. The portion of the complaint regarding care, protection and guidance was determined to be not valid. Standard #: 22VAC40-185-340-F
Complaint related: Yes
Description: Based on observation, the facility failed to ensure that children under the age of 10 were always within actual sight and sound supervision of staff. Evidence: 1) Staff A was observed in the foyer outside her classroom with one infant, leaving a second infant alone in the classroom for approximately 2-3 minutes. 2) In the preschool classroom, two children were observed outside on the deck alone. Approximately 2-3 minutes later, Staff B joined the children outside, leaving 16 children inside the classroom without sight and sound supervision. Another staff member was positioned in the cubby room and not able to provide sight and sound supervision to the children remaining in the classroom. 3) At approximately 12:30 pm, two children were observed leaving the main classroom and entering the cubby area out of sight and sound supervision of staff. Staff C joined the children approximately one minute later.

Plan of Correction: When 3 staff are in children's house, there will always be someone in the cubby room and the main room. If a third person is available, the deck will be available to the children. We will install a childproof lock to ensure an adult opens the door to the deck.

Description: Based on observation, it was determined that not all medication was kept in a locked place using a safe locking method that prevents access by children. A bottle of Advil was observed in the unlocked cabinet over the sink.

Plan of Correction: Medication was removed that day.

Complaint related: No
Description: Based on observation, and an interview with the director, the facility failed to post the license at the public entrance. Evidence: The provisional license was not posted anywhere.

Plan of Correction: It is hanging by the front door in a frame.

Description: Based on observation, and an interview with the director, the facility failed to post the findings of the most recent inspection of the facility (dated 12/17/18.) Evidence: The most recent inspection findings were not posted.
