Technical Assistance:
**Discussed with the director to clarify the reunification with parents and children after an emergency event policy.

Comments:
A Subsidy Health and Safety inspection was conducted today at 8:00am to 12:00pm to review supplemental health and safety requirements. There were 129 children under the supervision of 21 staff. Children were observed arriving to care, playing individually and in groups, getting ready to play outside, eating breakfast and circle time. There were medications on site today. There was a sufficient supply of books, toys, and materials that were developmentally appropriate for the children in care. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: 22VAC40-665-540-A
Description: Based on review, the facility did not have documentation of TB for one staff member at the time of employment or prior to coming into contact with children. Evidence: Reviewed 10 staff records and found that 1, Staff 6 did not have documentation of a TB screening.

Plan of Correction: Staff will bring documentation of TB screening.

Standard #: 22VAC40-665-770-B-5
Description: Based on review, the facility did not have documentation of continuity of operations procedures to ensure that essential functions are maintained during an emergency. Evidence: Reviewed the facility's records and identified through staff statements that they did not have a policy for continuity of operations.

Plan of Correction: Will add continuity of operations to the emergency preparedness plan.

Standard #: 22VAC40-665-780-A-3
Description: Based on review and staff interviews, the facility did not conduct a lockdown drill within the last 12 months. Evidence: Reviewed the emergency drill log and identified through staff comments that they have not conducted a lockdown drill within the past 12 months.

Plan of Correction: Will conduct a lockdown drill with staff and children.
