22VAC40-665 STAFF QUALIFICATIONS & TRAINING

Comments:
Health and Safety inspection was conducted to review supplemental health and safety requirements. There were 61 children in care with nine staff. Children present ranged in age from 16 months through 11 years old. Children were observed in teacher and self-directed activities. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: 22VAC40-665-540-A
Description: Based on record review and interview the center failed to ensure that staff be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department. Evidence: 1 - The record for staff 2 does not contain documentation of a tuberculosis screening. a. Staff 2 has a documented date of hire as February 28, 2019. b. Staff 2 was present providing direct care to children during the inspection. 2 - Staff 1 confirmed that there is not documentation of tuberculosis screening on file for staff 2.

Standard #: 22VAC40-665-550-2
Description: Based on record review and interview the vendor failed to inform the department's representative as soon as practicable, but not more than two business days, of any injury to a child while under the vendor's supervision when a referral is made for treatment from a medical professional. Evidence: 1 - The vendor did not inform the department's representative that, while in the care of the vendor, on January 28, 2019, child 1 sustained a head injury and was seen by a medical professional on the same day. 2 - Staff 1 stated that she did not inform the department of the incident because she was not aware she needed to do so.
