Comments:
An unannounced monitoring inspection was conducted on 4/3/19 from 10:10am -12:15pm. When the Licensing Inspector arrived there were 87 children ages two months old to five years old in care with 16 staff. Children were observed participating in various activities in the classrooms, playing outside, eating lunch and during rest period. Records were reviewed for nine children and ten staff. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-130-B
Description: Based on a review of nine children's records, it was determined that the facility did not ensure that additional immunizations are obtained once every six months for children under the age of two years old. Evidence: 1. The record for child #2 (23 months old) contained an immunization record that was dated 7/13/17, which is from when the child 13 months old. 2. Staff #11 (Program Director) reviewed the record for child #2, and confirmed that additional immunizations records were not obtained every six months for child #2.

Plan of Correction: The facility responded: The parent of the child #2 was contacted immediately during the inspection for an updated shot record. Management will continue to remind parents that whenever their child receives new immunizations, they are responsible for submitting copies of them to the office.

Standard #: 22VAC40-185-60-A
Description: Based on a review of nine children's records and interview, it was determined that the facility did not ensure that they maintain and keep at the center a complete record for each child enrolled that contains all required information. Evidence 1. The record for child #1 did not contain the name, address and phone number for the second emergency contact. 2. Staff #11 (Program Director) confirmed that the record for child #1 did not contain all of the required information.

Plan of Correction: The facility responded: The parents of child #1 will be asked to provide the missing information to make the record complete. All new children will have their record reviewed prior to enrollment.

Standard #: 22VAC40-185-270-A
Description: Based on observation, it was determined that the licensee did not ensure that all areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: 1. There were two seats on the center's bus where the seat cover was torn and the cushioning was exposed. 2. Staff #11 (Program Director) confirmed that two seats on the bus were not currently in a safe and operable condition.

Plan of Correction: The facility responded: The seats will be repaired by stitching them together. Staff will be reminded to report when anything is not in an operable condition.
