Comments:
An unannounced monitoring inspection began on March 6,2019. There were 61 children in care ages 16 months through 11 years old with nine teaching staff and the program director. Children and staff records were reviewed. A walking tour of the center inside and outside was completed. Required postings, annual inspections, emergency evacuation drills first aid kit, transportation and medication were reviewed. Children were observed in teacher and self-directed activities. Violations in the areas of administration, physical plant, programs, special care provisions and emergencies and background checks were identified and appear on the violation notice. The violations were reviewed with the program director at the conclusion of the inspection.

Standard #: 22VAC40-185-60-A
Description: Based on record review and interview the center failed to ensure that each child?s record contains all required information. Evidence: 1 ? The record for child 1 does not include documentation that proof of identity was viewed within seven days of enrollment. Child 1 has a documented first date of attendance as February 8, 2019. 2 ? Staff 1 confirmed that there is not any documentation that proof of identity was viewed for child 1.

Plan of Correction: Management will insure children?s proof of identity Will be established with in the 7 days of enrollment

Standard #: 22VAC40-185-270-A
Description: Based on observation and interview the center failed to ensure that areas and equipment are maintained safe and operable condition. Evidence: 1 ? There are areas of peeling paint on the wooden support posts for the swings within reach of children. 2 ? There is a sharp piece of metal on a chair that is on the playground within reach of children. 3 ? There are protruding bolts on the playground gates that have six to eight threads exposed within reach of children. 4 ? Children were observed using the playground during the inspection.

Plan of Correction: 1. Swings Area will receive a fresh coat of paint 2. Chair was removed from playground the evening of 03-06-2019 3. Bolts will be tightened to insure children?s safety

Description: Based on observation and interview the center failed to ensure that S-hooks where provided are not open more than the thickness of a penny. Evidence: 1 ? There are swings on the preschool and school age playground that have S-hooks that are open more than the thickness of a penny. a. The school age playground a two bay swing with four S-hooks open more than the thickness of a penny. b. The preschool playground has a three bay swing with three S-hook open more than the thickness of a penny. 2 ? Staff 2 confirmed that the S-hooks on the swings are open more than the thickness of a penny.

Description: Based on observation and interview the center failed to ensure that staff follow hand washing requirements. Evidence: 1 - Staff 3 did not wash her hands between diaper changes of children 2, 3, and 4. a. Staff three wore the same gloves for all three diaper changes. b. Child 3 had a BM diaper change. 2 ? Staff 3 did not have the children wash their hands after each diaper change. 3 ? Staff 3 confirmed that she did not wash her hands or the children?s hands between the three diaper changes. She further confirmed that she did not change her gloves between diaper changes.

Plan of Correction: Management will review and train on proper Diapering, Hand washing for staff and children and Glove changing between diapering

Standard #: 22VAC40-185-500-B
Description: Based on observation and interview the center failed to ensure that the diaper changing surface is cleaned and sanitized between each diaper change. Evidence: 1 ? Staff 3 did not clean or sanitize the diaper changing surface after changing the diapers of children 2, 3 and 4. a. Child 3 had a BM diaper change. The diaper changing surface was not cleaned or sanitized before child 4 was placed on the changing table. 2 ? Staff 3 confirmed that she did not clean or sanitize the diaper changing surface after each of the three diaper changes.

Plan of Correction: Management will review and train on proper cleaning and disinfecting of changing area After each diaper change.

Standard #: 22VAC40-185-540-C
Description: Based on review of first aid kit and interview the center failed to ensure that first aid kits on vehicles used to transport children in care contain all required items. Evidence: 1 ? The first aid kit on the van used to transport children in care, during the inspection does not have scissors, thermometer or triangular bandages. 2 ? Staff 4 confirmed that the items were not present in first aid kit, on the van she used to transport children in care, during the inspection.

Plan of Correction: First Aid Kits will be reviewed on a monthly basis. Items will be replenished as needed.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review and interview the center does not have a central registry finding within 30 days of employment. Evidence: 1 ? The record for staff 5 does not contain documentation that a central registry finding was obtained within 30 days of employment. a. The documented date of hire for staff 5 is April 10, 2018. 2 ? Staff 1 confirmed that the record for staff 5 does not contain documentation of a central registry finding.

Plan of Correction: Management has implemented a check list to insure documents are updated as needed. Check list will be checked on monthly bases. New hires will be updated upon receiving documentation until all items are completed
