An unannounced monitoring inspection was completed on April 8, 2019 from approximately 3:15p.m. to 5:00p.m. Ten children were in care during the inspection. Staff and children were observed in one classroom space and outside with adequate staff ratios. Children and staff were observed engaging in activities to include, snack time, completing homework, outside play, reading books,the toileting procedure and departure from the program. Children?s equipment, learning materials, classroom and outdoor space, rest rooms, the vehicle used for the center's transportation and the first aid and emergency supplies were inspected. Five children?s records, two staff records, annual fire and health inspections, fire drill and shelter in place logs, daily health observation certifications, injury reports, daily attendance, the emergency preparedness plan, written playground safety proceudres and required postings were reviewed. The center does not administer medications. The Owner/Director, was available for interview and the inspection and was present at the exit interview at which time inspection findings were reviewed. There were two citations for violations of the Standards for Licensed Child Day Centers. See the violation notice on the Department?s public web site for violations of the Standards.
Standard #: 22VAC40-185-580-C Description: Based on observation and interview, the licensee failed to ensure there was a list of the names of the children being transported in the center?s van. Evidence: 1. Upon observation of the center?s van used to transport children, a list of seven children?s names was observed. In an interview with Administrator #1 it was reported that twelve children are enrolled in the program and that this list does not include all the children who are transported daily. Plan of Correction: The Owner/Director will create an updated list of the children and place it in the van's binder.
Standard #: 22VAC40-80-120-E-2 Description: Based on observation, the licensee failed to post the most recent inspection of the facility. Evidence: 1. Upon observation, the findings of the most recent inspection were not posted at the center. 2. Administrator #1 acknowledged these findings were not posted. Plan of Correction: The Owner/Director will create a sign to direct the public that the findings are in an accessible folder for review.
