Technical Assistance:
Provided consultation on the requirement to take all children outside daily, weather and air quality permitting. One hour per day is required for toddlers and preschool aged children.

Comments:
An unannounced monitoring inspection was conducted from 1:20 -3:20 pm with the center co-owner and staff. There were 22 children, ranging from eight months to five years of age, present with four staff supervising. Children were observed napping, and playing quietly on their cots. Infants were observed eating, resting and playing with toys. Five children's files were reviewed. Four staff files were reviewed. The emergency escape drill log was reviewed and in compliance. Required information was posted. The playground was observed and is off limits until maintenance is completed. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on a review of five children's files, it was determined that the facility did not obtain documentation that each child has received a physical examination before the child's first attendance or within one month after attendance. Evidence: The file for child #2 (date of first attendance: 7/9/18) did not have documentation of a physical examination.

Plan of Correction: BBDA will ensure children physicals are received within a month of enrollment. Parents will receive notice that child will not be able to return without a complete file.

Standard #: 22VAC40-185-70-A
Description: Based on a review of four staff files, it was determined that the facility did not have all required information in each staff file. Evidence: For staff A (date of hire: 10/22/2018) there was no documentation of 1) job title, 2) name, address and telephone number for an emergency contact, 3) education required for the job position, or 4) two references as to character and reputation as well as competency before employment. For staff B (date of hire: 05/18/2018) there was no documentation of 1) job title, 2) name, address and telephone number for an emergency contact, or 3) two references as to character and reputation as well as competency before employment. For Staff D (date of hire: 2/18/2019) there was no documentation of 1) job title, 2) name, address and telephone number for an emergency contact, or 3) two references as to character and reputation as well as competency before employment.

Plan of Correction: Files will be adequately labeled to ensure Licensing Inspector is able to easily identify requirements of the file.

Standard #: 22VAC40-185-80-A
Description: Based on a review of the written attendance record and an interview with staff, it was determined that the facility did not ensure all children were included on the written attendance record. Evidence: The written record of attendance in the preschool room was last used on 3/6/19. There were 13 children present in the class with no attendance record.

Plan of Correction: Attendance records will be kept, maintained and posted daily. The Supervisor will inspect logs daily. Date to be corrected: Immediately

Standard #: 22VAC40-185-210-A
Description: Based on a review of four staff files, it was determined that the facility did not have documentation that all staff meet the qualifications of program leader. Evidence: The file for staff A, who serves as lead teacher in the toddler room, did not include documentation of at least high school or the equivalent, or a minimum of 24 hours of training related to the care of children. The file for staff D, who serves as lead teacher in the 2's class, did not include documentation of a minimum of 24 hours of training related to the care of children.

Plan of Correction: Files will be adequately labeled to ensure Licensing Inspector is able to easily identify requirements of the file.

Standard #: 22VAC40-185-240-A
Description: Based on a review of four staff files, it was determined that the facility did not have documentation that all staff received orientation training that includes job responsibilities, policies/procedures, playground safety procedures, recognizing child abuse and neglect, and confidentiality by the end of their first day of assuming job responsibilities. Evidence: Staff A (date of hire: 10/22/2018) and staff D (date of hire: 2/18/2019) did not have documentation of orientation training.

Plan of Correction: Files will be adequately labeled to ensure Licensing Inspector is able to easily identify requirements of the file.

Standard #: 22VAC40-185-270-A
Description: Based on observation, not all areas of the outside playground were maintained in a clean, safe condition. Evidence: In the corner of the playground, the wire fencing has pulled away from the wooden fence leaving a gap large enough for a child to crawl through. There were various pieces of trash on the playground including a rusty paint can and small bits of glass. There were several exposed tree roots causing a tripping hazard.

Plan of Correction: BBDA will ensure outdoor playground area is safe and well maintained. At time of inspection the playground area was closed due to the harsh winter weather conditions. Spring maintenance of outdoor play area was conducted by parents and staff on March 29 and April 8, 2019.

Standard #: 22VAC40-185-290-3
Description: Based on observation, the facility failed to ensure all electrical outlets had protective covers. Evidence: There was an uncovered electrical outlet in the infant room.

Plan of Correction: BBDA will ensure all electrical outlets have protective covers. Date of correction: immediately

Standard #: 22VAC40-185-340-A
Description: Based on observation, the facility failed to ensure the children's care, protection and guidance. Evidence: 1) There were 13 preschool children left alone in a classroom during naptime; 5 children were wide awake and playing on their cots. 2) The LI observed all four classrooms out of ratio and without sight and sound supervision at different times during the inspection. Two teachers left their classrooms unattended when they opened the door for the LI, one teacher left the classroom unattended in order to check on the children left alone in another room, and the owner was not present at the time the LI arrived; staff contacted him and he returned.

Plan of Correction: A Floater will no longer be able to walk between classrooms to monitor children in 5 minute increments during nap/quiet time to relieve staff for lunch breaks. Additional staff will be hired to manage call outs and/or extended leave. BBDA will ensure children?s care, protection, and guidance standards are met. Date to be corrected: Immediately Announcement for hiring is open continuously.

Standard #: 22VAC40-185-340-F
Description: Based on observation, the facility failed to ensure that children under the age of 10 are always within actual sight and sound supervision of staff. Evidence: 1) When the LI arrived, two staff came to open the door. There were two infants left alone in a classroom (one was awake in a crib drinking a bottle and the other was sleeping in a crib,) and two toddlers (ages 16 months and 22 months,) both awake lying on cots, left alone in a classroom without supervision for a period of 2-3 minutes. 2) From 1:20 pm until approximately 2:20 pm there were 13 children, ages 3-5 years, alone in a classroom without any adult supervision. Eight of the children were resting on cots and five were awake.

Plan of Correction: A Floater will no longer be able to walk between classrooms to monitor children in 5 minute increments during nap/quiet time to relieve staff for lunch breaks. Additional staff will be hired to manage call outs and/or extended leave. BBDA will ensure children under the age of 10 are always within actual sight and sound supervision of staff. Date to be corrected: Immediately Announcement for hiring is open continuously.

Standard #: 22VAC40-185-350-E-4
Description: Based on observation, the facility failed to maintain the ratio of one staff member for every ten children ages three years to five years. Evidence: There were 13 children, ages 3-5, left alone in a classroom during naptime for approximately 1 hour.

Plan of Correction: A Floater will no longer be able to walk between classrooms to monitor children in 5 minute increments during nap/quiet time to relieve staff for lunch breaks. Additional staff will be hired to manage call outs and/or extended leave. BBDA will ensure the ratio of one staff member for every 10 children ages three to five. Date to be corrected: Immediately Announcement for hiring is open continuously.

Standard #: 22VAC40-185-500-A
Description: Based on observation, the facility failed to wash children's hands with soap and running water after toileting (diapering). Evidence: after diapering a toddler, a staff person used a baby wipe to clean the child's hands and then placed the child on their cot.

Plan of Correction: Staff was reminded to wash hands with soap and water after diapering.

Standard #: 22VAC40-185-570-B
Description: Based on observation, the facility failed to ensure that infants who cannot hold their own bottles are held when fed. Evidence: One infant was observed lying in a crib drinking a bottle. Staff propped a second infant's bottle with a blanket while the child was lying in a bassinet.

Plan of Correction: Staff was reminded of infant care policies/procedures/standards. A verbal warning was given and official documentation was placed in Staff file. Staff was also removed from infant care. BBDA will ensure infants who cannot hold their bottles are fed. Date to be corrected: 4/3/19. Infant/Toddler training through Childcare aware to be provided 4/22/19 at 10am.

Standard #: 22VAC40-185-570-E
Description: Based on observation, and an interview with staff, it was determined that not all infant formula/breast milk was refrigerated/labeled/dated. Evidence: 1) There was a bottle in the refrigerator that was not labeled or dated, 2) There was breast milk observed sitting in a child's diaper bag (at 1:30 pm) that was not dated, labeled or refrigerated.

Plan of Correction: Staff was reminded of infant care policies/procedures/standards. A verbal warning was given and official documentation was placed in Staff file. Staff was also removed from infant care. BBDA will ensure all infant formula/breast milk is labeled, dated and refrigerated. Date of correction: Immediately. Infant/Toddler training through Childcare aware to be provided 4/22/19 at 10am.
