Violations:
Standard #: 22VAC40-185-350-C
Complaint related: No
Description: Based on information received and discussions with administrative staff the facility failed to maintain compliance with this standard related to ratios as required. Evidence: On 4-2-19 there were 16 children, preschool-school aged, in care with only 1 staff. The required ratio for that grouping of children was 1 staff to 10 children. The staff was out of ratio from approximately 7:45am until 8:15am.

Plan of Correction: The RD, PD are working to ensure that staff are scheduled appropriately for maintaining morning ratios. HR is in the process of hiring staff to ensure that positions are filled and that ratios can be met at any given time.
