63.2 Child Abuse & Neglect

Comments:
An unannounced mandatory inspection was conducted between 9:05am and 11:50am today. There were 14 children present with 3 staff, well with staffing guidelines. The children were engaged in centers, playing with puzzles and singing. Positive interactions were observed between the children and the staff. A complete inspections was done of the physical plant. In addition, staff/children's records, evacuation drills, safety procedures, attendance and safety supplies were reviewed. Areas of non compliance were listed on the Violation Notice. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children's files, the center failed to maintain and keep at the center a separate and complete record for each child enrolled. Evidence: 1) Child #1 and child # 3's files were missing the first emergency contact address.

Plan of Correction: I will contact parents and get information and place in children;s files.

Standard #: 22VAC40-185-70-A
Description: Based on record review, two of three staff records are incomplete. Evidence: 1) Staff 2 and #3's files do not have documentation of 2 employment references.

Plan of Correction: We will get references and place them in our files.

Standard #: 22VAC40-185-290-3
Description: Based on observation, the center failed to ensure that electrical outlets were to have protective covers that are of a size that cannot be swallowed by children. Evidence: 1) There was one protective cover missing in the entry hallway. 2) There was one protective cover missing in the 2 year old classroom. 3) There was one protective cover missing in the first bathroom.

Plan of Correction: I will replace them.

Standard #: 22VAC40-185-320-B
Description: Based on observation, the center failed to ensure that each rest room area for the children shall be equipped with toilet paper and paper towels. Evidence: Bathroom #1 did not have toilet paper and Bathroom #1 and Bathroom #2 did not have paper towels.

Plan of Correction: Replaced during inspection.

Standard #: 22VAC40-185-560-G
Description: Based on observation, the center failed to ensure that when food if brought from home it was not clearly labeled with the child's name and date. Evidence: 1) 3 lunches in classroom one were not labeled with the children's names. 2) 5 lunches in classroom two were not labeled with the children's names.

Plan of Correction: Will label lunches when parents forget.
