Technical Assistance:
Inspector spoke with the Director about cribs during this inspection.

Comments:
An unannounced monitoring visit and facility tour was conducted from 10:25 am-12:00 pm. At the time of inspectors' arrival there were 51 infant to preschool aged children in care with 10 staff members and 1 administrator. Children were observed participating in instructional time in the classroom, outdoor and indoor play, transitioning between activities, and toileting and hand washing. A sample of 5 children's records and 5 staff records were reviewed. Injury reports were also reviewed. The Director reported that no medications are given at the center at this time. Areas of noncompliance are detailed on the Violation Notice and were discussed with the Director at the conclusion of this inspection.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on record review and interview it was determined the center did not ensure that all staff resubmitted TB test results every 2 years from the date of the first initial screening or testing. Evidence: 1. The record for staff #4 did not contain updated TB test/screening results. 2. The documentation in staff #4's record was dated 3/22/17. 3. The Director confirmed that updated documentation was not available during this inspection.

Plan of Correction: Staff #4 will get updated TB testing or screening.

Standard #: 22VAC40-185-420-E-1
Description: Based on record review and interview it was determined the daily record kept for each infant in care did not contain all of the required components. Evidence: 1. Staff #3 provided inspector with the daily sheets utilized for infants in care. 2. The daily sheet provided did not include: -description of bowel movements -developmental milestones -for infants, who are awake and cannot turn over by themselves, the amount of time spent on their stomachs

Plan of Correction: Director will add the missing components to the daily sheet.

Standard #: 22VAC40-185-570-E
Description: Based on observation it was determined that the center did not ensure that all infant formula is dated and labeled with the child's name. Evidence: There were multiple bottles in the refrigerator in the infant room that did not contain names or dates.

Plan of Correction: Staff will ensure all infant bottles are labeled with the infant's name and the date.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review and interview it was determined the center did not ensure that each staff member had a Central Registry finding within 30 days of employment. Evidence: 1. The records for staff #3(hired 2/5/19), staff #4(hired 8/27/2018), and staff #5(hired 7/30/18) did not contain Central Registry findings. 2. The employee files contained both the National Background check results and the State Police record checks and the Director reported she had made a mistake and was sending for the State Police check and not the Central Registry Check.

Plan of Correction: Director will send off for the Central Registry checks.
