Comments:
The licensing inspector conducted an unannounced renewal inspection Wednesday, April 3, 2019 from 12:20pm to 2:50pm. The children were observed resting on cots during nap time. The school age children were observed eating lunch. Lunch today consisted of sloppy joe on bun, corn, applesauce and milk. The menu was posted. The infants were observed playing with toys, being changed and being held. All classrooms and playgrounds were inspected today as well as the facility?s mini buses. Eight children?s records and eight employee records were reviewed during this inspection. The center administers medication; however they did not have any being given at this time. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 03/11/2019 Last shelter-in-place drill: 03/19/2019 Last fire inspection: 12/14/2018 Last health inspection: 03/05/2019 Total enrollment today: 80 Today, the following child to staff ratios were observed: Infants ?13:4 Toddlers ?13:2 Twos? 16:2 Threes ?15:2 Pre-K-13:1(rest time ratio) School Age ? 10:1 The violations from the previous inspections were checked for corrections. There were two repeat violations found during today?s inspection. The Department has not yet received a complete renewal application and annual fee from the center. The Department must receive a complete renewal application and fee prior to expiration of the license on 05/20/2019 or the Department cannot renew the license. If you have any questions about this inspection, please contact the licensing inspector, Roshon Casey, at (804) 662-9760. PROVIDER: Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Description: Based on a review of children's records on 04/09/2019, the center did not ensure that there was a name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached; EVIDENCE: The record of Child #3 did not contain an address for the second emergency contact listed.

Description: Based on observation on 04/3/2019, the center did not ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards. EVIDENCE: The mulch on the infant/toddler playground measured less than six inches.
