Comments:
Today an unannounced monitoring inspection was conducted starting at 2:30pm and ended at 4:15pm. There were 44 children present today with 4 staff. Children were observed arriving from school, playing various table top activities, playing kickball and eating snack. Medications with authorizations were reviewed. Medication Administration Training (MAT) and First Aid/CPR certifications were checked today. Staff records are reviewed annually in the administrative office. Standard #: 22VAC40-185-60-A
Description: Based on interview, the center does not have a separate record for each child enrolled. Evidence: During today's inspection, center staff was unable to access CASA's online record system due to internet connection issues. 72 out of 72 children's records were not accessible during the inspection.

Plan of Correction: An email was sent to the supervisor letting them know that the system was not working.

Standard #: 22VAC40-185-70-A
Description: Based on record review and interview, emergency contacts were not accessible for one staff member. Evidence: The emergency contact information for Staff #3 is not accessible during today's inspection.

Plan of Correction: I will have her fill this out today.
