You are reminded that classroom not occupied but accessible to children in care must comply with securing cleaning agents.

Comments:
An unannounced monitoring inspection was conducted this date. A total of 81 children with 14 staff present during the inspection. Walk thru of all classrooms completed. Two classrooms were not occupied at this time. Preparation of lunch was observed. A total of 8 children's records and 6 staff records were reviewed today. Topical ointment authorizations reviewed and found complete. Previous violations were reviewed from the November inspection and one repeated uncorrected violation found. Violations:
Standard #: 22VAC40-185-70-A
Description: Based on staff record review, lead qualifications for one staff was not found. Evidence: Staff #9 is in a lead position in the PreK classroom, documentation of lead qualifications were not found in the staff record.

Plan of Correction: We will require these documents by the end of the week and will forward to licensing.

Standard #: 22VAC40-185-270-A
Description: Based on observation of the inside and outside of the center, some areas were not maintained in a safe and operable condition. Evidence: The wallpaper located in the beginners classroom to the left of the perimeter door is peeling in two areas. The bolts on the boundary bumper on the Pre-K playground are protruding approximately 2 1/2 inches from the bumpers presenting a tripping hazard. A screw located in the molding of the Beginners left side of the wall is protruding from the molding presenting a hazard. Repeated from the November inspection: The bottom right corner of the perimeter door off the Toddler classroom is rusted and has peeling paint. This remains uncorrected from the last inspection.

Plan of Correction: We will repair all of these areas.

Standard #: 22VAC40-185-490-E
Description: Based on observation of the main infant classroom, one crib mattress was contaminated with bodily fluids and was not cleaned and sanitized properly. Evidence: Licensing Inspector observed Staff #1 changing a soiled crib sheet after an infant vomited. Staff #1 failed to clean the mattress with soap and water. Staff #1 used a cloth with bleach solvent to wipe the mattress.

Plan of Correction: We will retrain staff of this process of first cleaning and then sanitzing these surfaces.

Standard #: 22VAC40-185-500-A
Description: Based on observation of the Intermediate classroom, staff failed to follow hand washing procedures. Evidence: Licensing Inspector observed Staff #2 wipe a child's nose and failed to wash hands afterwards.

Plan of Correction: We will remind all staff of the hand washing procedures.

Standard #: 22VAC40-185-500-B
Description: Based on observation of the main infant classroom and staff statements from the toddler classroom diapering procedures are not followed. Evidence: The diaper mat located in the infant class had a crack on the side of the mat and is no longer non-absorbent. Staff in the Toddler classroom stated that both the diaper surface and tables are not sanitized with a bleach and water solvent. Staff stated they are cleaning these surfaces with a Lysol and water solvent and not sanitizing after cleaning the surfaces.

Plan of Correction: We will replace this mat and remind staff to report any cracks in the diaper mats to management. We will retrain all staff that these surfaces must be first cleaned with soap and water and then sanitized with bleach and water.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation of the center, the findings of the most recent inspection was not clearly posted. Evidence: The most recent violation notice from November 2018 was covered by another document.

Plan of Correction: We will remove this other document so the violation notice is visible.
