Comments:
A Subsidy Health and Safety inspection was completed on April 1, 2019 from approximately 11:45a.m. to 2:15p.m. Fifty-nine children were in care during the inspection. Staff and children, ages infant to school age, were observed in five classrooms with adequate staff ratios. Children and staff were observed engaging in activities to include activities for infants and lunch and nap time. Children?s equipment, learning materials, outdoor and classroom space, rest rooms, the center's vans and the first aid and emergency supplies were inspected. Five children?s records, five staff records, annual fire and health inspections, fire drill, shelter in place and lock down drill logs, daily health observation certifications, injury reports, daily attendance, the emergency preparedness plan and required postings were reviewed. The Director was available for interview and the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Standard #: 22VAC40-665-520-B-11
Description: Based on a review of children's records and interview, the licensee failed to maintain a written allergy care plan for each child with a diagnosed food allergy. Evidence: 1. A list of the children with food allergies was reviewed. None of these children's record's contained a written allergy care plan to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction. 2. Administrator #1 acknowledged the center has not obtained written allergy care plans for children with diagnosed food allergies.

Plan of Correction: The Owner/Director will make sure that allergy plans for children are written up.

Description: Based on a review of records, the licensee failed to include a component in the emergency preparedness plan regarding continuity of operations procedures to ensure that essential functions are maintained during an emergency. Evidence:1. A review of the emergency preparedness plan revealed no component as to how continuity of operations procedures to ensure that essential functions are maintained during an emergency will occur. 2. Administrator #1 acknowledged this was not included in the emergency preparedness plan.

Plan of Correction: The Owner/Director will add the missing component to the emergency plan.
