Reviewed Standard 440.J - Crib spacing when cribs are occupied with an infant. Reviewed Standard 320.E - Centers shall have one sink and one toilet per 20 preschool children.
An unannounced monitoring was conducted on 04/12/2019 from 10:45am - 1:55pm. At the time of the tour there were 33 children in care with seven teaching staff and three administrative staff. The children ranged in age from infant - four years. The children were observed during morning program time, lunch and nap. Records were reviewed for seven staff and nine children. The violation is listed on the violation notice and was reviewed with the program director at the conclusion of the inspection.
Standard #: 22VAC40-185-140-A Description: Based on record review and interview, the center failed to ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance. Evidence: 1. There is no documented physical on file for child 1 who has an enrollment date of 12/12/2018. 2. There is no documented physical on file for child 2 who has an enrollment date of 01/03/2019. 3. The center director confirmed that child 1 and child 2 did not have a physical on file. Plan of Correction: We have requested physicals from both parents and have had difficulty with obtaining the forms from them. We also have a record of the constant communication with them regarding the requests. On Friday afternoon of April 12th, we have requested and received the physical for Child 2. For Child 1, the parent has an appointment in May for her child to have her physical completed. For all future enrollments, we will request for physicals to be submitted upon enrollment.
