Comments:
An unannounced renewal inspection was completed on 04/15/2019 from 10:30 A.M.-3:45 P.M. At the time of the inspection 55 children were in care with eleven staff and one administrator present. Six children records, seven staff records, two over the counter diaper ointments and documentation, six injury reports, emergency plan, first aid kits, emergency supplies, fire inspection, health inspection, playground safety plan, injury prevention plan, emergency plan, playground, and posted required information were reviewed. Children were observed during infant feeding, free play, behavioral guidance, handwashing, diapering, outdoor play time, story time and discussion, lunch time, departure, and naptime. Standard #: 22VAC40-185-160-C
Description: Based on review of staff files, the center did not ensure that an updated tuberculosis statement was obtained at least every two years from the date of the first initial screening or testing. 1. Staff #6's last documented tuberculosis screening was dated 04/14/2017. 2. Staff verified the last documented tuberculosis screening.

Plan of Correction: Administration had given the staff member the paperwork to get the tuberculosis screening completed, and the staff had not followed thru with the update. Administration will meet with the staff member and request and appointment be made immediately.

Standard #: 22VAC40-185-550-M
Description: Based on review of injury reports, the center failed to include all required information on the injury reports. Evidence: 1. Injury report dated 03/07/2019 was missing the time and method parent was notified. 2. Injury report dated 02/18/2019 was missing the time parents notified, and a second signature from staff or a parent. 3. Staff verified that the reports were missing all the required information.

Plan of Correction: Administration will review the requirements for completing injury reports with all staff during a staff meeting.

Standard #: 63.2(17)-1721.1-B-2
Description: Based on a review of board member background checks, the center failed to ensure that all current board members had fingerprints submitted by 09/30/2018. Evidence: 1. Board Member A's fingerprint check was dated 01/09/2019. Board Member A has been on the board of the organization since it opened in 1999. 2. Staff verified the date of the fingerprints and the date Board Member A joined the organization.

Plan of Correction: In the future any new board officers will obtain all background checks within 30 days of taking office.
