An unannounced Monitoring Inspection was initiated in the center on April 9, 2019 and completed on April 17, 2019 in regards to a self-reported incident received by the department related to programs. An inspection was conducted at the facility, that included interviews with staff and a review of center documentation. Interviews with other parties involved (collateral) were also conducted. The area of non-compliance was noted on the violation notice. The results of this inspection were reviewed with Center Management during the exit interview. Standard #: 22VAC40-185-410-1 Description: Based on interviews and center documentation, the licensee did not ensure there was not a threat of physical punishment for enrolled children. Evidence: On April 1, 2019 multiple individuals reported hearing staff #1 threatening to get a belt after a child #1, age 3. Plan of Correction: The Center Director terminated staff #1 the next day.
