63.2(17) License & Registration Procedures

Comments:
Licensing Inspector arrived at 12:00 and departed at 1:30 PM to conduct an unannounced renewal inspection. At the time, there were six children (aged 12 months through four years in care with one staff member. Children were observed napping. The sample size was three staff and six children.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on a review of six children's records, it was determined that the facility did not ensure that each child had the results of a physical examination on file within 30 days of enrollment. Evidence: Child #3 (date of enrollment 1/22/18) did not include the results of a physical examination.

Plan of Correction: Center management will ensure that all children have physicals on file within 30 days of enrollment.

Standard #: 22VAC40-185-350-C
Description: Based on observation, it was determined that the facility did not ensure that when children are in ongoing mixed age groups, the staff-to-children ratio for the youngest child in the group shall apply. Evidence: At the time of the inspection, there were six children sleeping in the infant area with one staff member. The youngest child was aged 12 months. Therefore, the ratio should have been 1 staff for every four children and an additional staff member was needed.

Plan of Correction: Center management will ensure that there are adequate staff present for the number of children in care.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of three staff records, it was determined that the facility did not ensure that all staff had a completed search of the central registry on file within 30 days of employment. Evidence: 1. The record for staff #3 did not include the results of a search of the central registry. 2. Staff #1 had reported at the December 2018 visit that staff #3 had been employed "a couple of weeks" and at this inspection verified her continued employment. This means that staff #3 had been employed over 30 days.

Plan of Correction: Staff #3 will obtain a search of the central registry and results will be placed in records. Moving forward all staff will obtain a search of the central registry within 30 days of employment or volunteering.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on review of three staff records, it was determined that the facility did not ensure that all staff completed fingerprinting prior to employment. Evidence: 1. The record for staff #3 did not include the results of fingerprints being completed. 2. Staff #1 had reported at the December 2018 visit that staff #3 had been employed "a couple of weeks" and at this inspection verified her continued employment.

Plan of Correction: Staff #3 will obtain fingerprints and results will be placed in records. Moving forward all staff will obtain fingerprints before employment or volunteering.
