Technical Assistance:
Today we discussed playgrounds and over-the-counter diaper ointment.

Comments:
An unannounced monitoring inspection was conducted today from 12:50pm through 2:30pm. There were 54 children in care with ten staff. Children present ranged in age from four months through five years old. Children and staff records were reviewed. Children were observed in teacher self-directed activities. A walking tour of the center inside and outside was completed. Emergency evacuations, first aid kit, annual inspections and medication were reviewed. Violations regarding physical plant, unlocked chemicals, infant sleep space and diaper changing were identified and appear on the violation notice. The violations were reviewed with the program director during an exit interview at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation and interview the center failed to ensure that areas are maintained in safe condition. Evidence: 1 - There are three broken metal hook in the three year old room. a, The hooks are located approximately two and half feet from floor and therefore, are within reach of children in care. b. The edges of the hooks are jagged to the touch. c. Children were in care in the room during the inspection. 2 - Staff 2 stated that the hooks were broken and needed to be removed.

Plan of Correction: The hooks have been removed. We also checked all other hooks and removed any that were not in good condition.

Standard #: 22VAC40-185-280-B
Description: Based on observation and interview the center failed to ensure that hazardous substances are kept in a locked place using a safe locking method that prevents access by children. Evidence: 1 - There is spray can of disinfectant spray on the hooks in the hallway between the four/five and three year old rooms. a. The spray can of disinfectant was within reach of children. b. Children were in care in both classrooms and the children passed through the hallway 2 - Staff 3 confirmed that there was a spray can of disinfectant on the hooks in the hallway between the four/fave and three year old rooms.

Plan of Correction: Corrected during the inspection. Staff will be reminded that all chemicals must be placed in the locked closet when not in use.

Standard #: 22VAC40-185-440-I
Description: Based on observation and interview the the center failed to ensure that cribs are placed where objects outside the crib are not within reach of infants or toddlers. Evidence: 1 - The crib child 1 (18 months old) was placed within reach of other crib mattresses and sheets. a. Staff 4 stated that the crib had to moved in order to to move it away from other occupied cribs. b. However, because of the all the toys and equipment stored and stacked in the infant sleep space staff were limited as to where the crib could be moved. c. The only option for moving the crib of child 1 away from the other occupied cribs was to place the crib within reach of another crib.

Plan of Correction: The sleeping area will be cleaned out and the space will only be used for cribs.

Standard #: 22VAC40-185-500-B
Description: Based on observation and interview the failed to ensure that diaper changing is completed on a diaper changing surface. Evidence: 1 - Staff 5 reported that wet pull-up diapers are changed in the bathroom while children are standing up and not a changing table or designated diaper changing surface. 2 - There are pull-up diapers stored in the bathroom next to the toilets.

Plan of Correction: We now take all children that are in pull-ups to the toddler changing area. The pull-ups have been removed from the bathroom and kept in individual cubbies.
