Comments:
Today an unannounced Monitoring Inspection took place between the hours of 10:15am and 11:50am. At the start of the inspection there were a total of 8 children with 2 staff; all ratios were incompliance. A total of 4 children?s files and 3 staff files were reviewed. The children were observed working in small groups, cutting and gluing for a craft, doing, playing with play dough and other organized activities. Today's lunch is turkey meatballs over rice with a tossed salad and ranch dressing, fresh fruit and milk. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records did not contain all required information. Evidence: Child #3 does not have a second emergency contact that is not a non-parent.

Plan of Correction: I will see the parents today and I will ask them for a second emergency contact.

Standard #: 22VAC40-185-510-J
Description: Based on observation, medication is not kept in a locked place. Evidence: There is a bottle of fever reducing medication in the cabinet under the staff bathroom sink. The cabinet is not locked and the door is open to the staff bathroom.

Plan of Correction: The medication belongs to the school for the staff. This was corrected during the inspection.
