Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 10:00 AM and departed at 1:40 PM. Twenty children were inc are at the start of the inspection. Morning snack, activities and nap time were observed. A sample size of five staff records and three children's records were reviewed. Medication administration and injury records were reviewed. An exit meeting was conducted with the program director prior to closure of the inspection.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based upon review of three children's records, the facility has not ensured that each child has a physical examination by or under the direction of a physician before attending or within 30 days after attending. Evidence: 1. The record provided for child 1 indicated a first date of attendance of 2/5/2019. The record did not include documentation of a physical examination. 2. Staff 1 verified that the record did not include a physical examination.

Plan of Correction: The facility responded with the following: The parent will be notified to provide documentation of a physical examination. All children's records will be checked to ensure that they contain documentation of physical examinations.

Standard #: 22VAC40-185-40-D
Description: Based upon observation and staff interview, the facility has not ensured that the license is posted in a place conspicuous to the public. Evidence: 1. The license posted by the front door expired on 3/15/2019. The current Provisional license was not posted. 2. Staff 1 acknowledged that the posted license was expired and posted the Provisional license during the inspection.

Plan of Correction: The facility responded with the following: The current license was posted during the inspection.

Standard #: 22VAC40-185-70-A
Description: Based upon review of staff records and staff interview, the facility has not ensured that each staff record contains all required information. Evidence: 1. According to staff 2, she left the employ of the facility and was re-hired. According to staff 7, she was on a leave of absence. The record provided did not include the termination date or the date of re-hire or documentation of a leave of absence status. 2. Staff 3 is employed at another site owned by the same Licensee. She was on duty with the children during today's inspection. The facility did not have the name, address and telephone number of an emergency contact for staff 3.

Plan of Correction: The facility responded with the following: 1. Documentation of the leave of absence of staff 2 will be clearly indicated in the record. 2. The emergency contact information was obtained during the inspection.

Standard #: 22VAC40-185-570-I
Description: Based upon observation and staff interview, the facility has not ensured that they have a one-day's emergency supply of disposable bottles, nipples and commercial formulas appropriate for the children in care. Evidence: 1. The emergency infant supplies were not accessible as the facility did not have the key to unlock the emergency supply bin. 2. Staff 1 verified that the key to the emergency supply bin was misplaced.

Plan of Correction: The facility responded with the following: The lock will be cut off of the emergency kit and replaced with a new lock.
