Comments:
An unannounced monitoring inspection was conducted on Tuesday, April 9, 2019 to determine the center's compliance with licensing standards. The inspector arrived at 9:50 am and concluded the inspection at 2:00 pm. The census for today?s inspection consisted of 89 children in the direct care of 16 staff. The center's director and assistant director assisted the inspector throughout the inspection. Upon arrival, the children were engaged in various activities. The children in the Discovery Preschool classroom were observed playing in different centers. The Waddlers were observed dancing, singing songs, and interacting with staff and peer. The Toddlers were playing with bubbles and singing songs. The Infants were observed resting and having tummy time. Diapering and feeding procedures were also observed. The Preschool children were observed during a group activity where they were practicing writing their names. The Pre-K children were observed in centers such as writing, home living, blocks, and later were observed as they cleaned up each center. The center has age and stage appropriate equipment and the classrooms are decorated with the children's artwork. The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, menu and various parent information. The center's first aid kit and non-emergency supplies were inspected and found complete. Transportation is provided and three buses were inspected today. The center's first aid kit and non-emergency supplies were inspected and found complete. Eight children's records and five staff records were reviewed during today's inspection. Emergency medications and authorizations were reviewed. Transportation is provided and three buses were inspected today. Health Inspection: 12/12/2018 Fire Inspection: 02/19/2019 The last documented evacuation drill was held on 03/25/2019. The last documented shelter-in-place drill was held on 09/17/2018. The monthly menu was posted and today's lunch consisted of: chicken, spaghetti, zucchini, pears, and milk. Standard #: 22VAC40-185-140-A
Description: Based on a review of eight children's records and interview on 04/09/2019, the center did not ensure that one child had a physical examination by or under the direction of a physician before the child's attendance; or within one month after attendance. EVIDENCE: 1) The record for Child #6 (DOE: 09/04/2018) did not contain a physical examination. 2) During interview, a member of management confirmed the physical examination report for Child #6 had not been received to date.

Plan of Correction: Per the Center: We are working with the parents in acquiring the missing physical.

Standard #: 22VAC40-185-60-A
Description: Based on a review of eight children's records, observation, and interview on 04/09/2019, the center did not ensure that one child's record contained all the required information. Evidence: 1) The record for Child #1 (DOE: 08/20/2018) did not contain written authorization for a prescription Epipen on supply for the child to be administered in case of an emergency.

Plan of Correction: The parents were contacted while the inspector was on site and requested to provide the completed written authorization for the prescription Epipen for the child.

Standard #: 22VAC40-185-280-B
Description: Based on observation at the center on 04/09/2019, the center did not ensure to keep all hazardous substances such as cleaning materials in a locked place using a safe locking method that prevents access by children. Evidence: 1) The licensing inspector observed a bottle of hand sanitizer on a counter top in the Preschool room. The hand sanitizer was not kept in a locked place and was accessible to the children in care.

Plan of Correction: Per the Center: We have reiterated the importance of locking up chemical substances with our teachers, and are checking the classrooms several times per day to ensure this is being done.

Standard #: 22VAC40-185-500-B
Description: Based on observation on 04/09/2019, the center did not ensure the diapering procedures were followed. Evidence: 1) The licensing inspector observed the trash can in the bathroom area used for diapering/potty training purposes in the Discovery Preschool classroom. Soiled diapers/pull-ups were observed inside the uncovered, trash can that was not foot operated. Soiled diapers/pull-ups should be disposed of in a leak proof, covered disposable system that is either foot operated or used in such a way that the staff member's hand nor the soiled diaper touches the exterior surface of the storage system.

Plan of Correction: Per the Center: We have placed a new trash can with a foot operated release in the Discovery Preshool restroom.

Standard #: 22VAC40-185-550-D
Description: Based on a review of documentation and interview on 04/09/2019, the center did not ensure that an evacuation drill was practiced monthly. Evidence: 1) The fire drill log did not reflect an evacuation drill for the month of January 2019. 2) During interview, a member of management confirmed an evacuation drill was not conducted for the month of January 2019.

Plan of Correction: Per the Center: We also did a tornado and fire drill for the month of April and we have created a new system for the future to help prevent missing any further drills.
