Comments:
An unannounced Monitoring inspection was conducted in the center on April 9, 2019. This Licensing Inspector observed 32 children present in two classrooms. Five staff members were present. Two staff members were with each group of children. As reported, there are currently 36 children enrolled in the program. The children were observed in each classroom as they participated in varied activities. Hand washing procedures and the lunch meal were also observed. A sample of 5 children's records was reviewed. Documentation of the center's annual fire and health inspections and first aid kits were reviewed in the center. Prescribed medications for 4 children were observed in the center. Dismissal procedures were observed in the late afternoon. Findings were reviewed with the center's lead teacher at exit. The on-site inspection was conducted from approximately 11:30am to 3:40pm. Standard #: 22VAC40-185-60-A
Description: Based on a sample review of children's records, the center failed to maintain the following information in each child's record: Name, home address and home phone number of each parent who has custody; and when applicable, work phone number and place of employment of each parent who has custody. Evidence: Information in 3 of 5 children's records was not complete: child #3: parent's work phone number; children 4. and 5. name, address, and phone number of each parent who has custody and the work phone number and place of employment, when applicable.

Plan of Correction: Children's records will be reviewed by designated staff (FDS), information will be obtained from each child's parent and documented in each record.

Standard #: 22VAC40-185-290-3
Description: Based on direct observations, the center failed to ensure all electrical outlets had protective covers that are of a size that cannot be swallowed by children. Evidence: The LI observed an electrical wall outlet within the reach of children under the window in classroom A. that did not have a protective cover. Staff covered the outlet during the inspection.

Plan of Correction: Staff will monitor outlets on-going to ensure protective covers are in place.

Standard #: 22VAC40-185-510-C
Description: Based on medications observed in the center, the center failed to ensure procedures for administering medication were in place to include: (3.) duration of the parent's authorization for medication, provided that it shall expire or be renewed after 10 work days, and (4.) include methods to prevent use of outdated medication. Evidence: The LI reviewed a sample of 4 children's medications in the center. A prescribed medication to be administered as needed for child a. noted an expiration date of 11/2018 and; the parental authorization to administer a medication to child d. did not indicate the start and end dates of the parent's authorization.

Plan of Correction: Staff will review and implement the center's medication policies to ensure any expired medication is returned to parent and required documentation for medication administration is current and complete. The child's parent will be notified to bring a current prescription to the center and the expired medication will be returned to the parent.
