A Subsidy Health and Safety inspection was conducted on April 22, 2019 to review supplemental health and safety requirements. New subsidy requirements became effective on October 17, 2018. The on-site inspection was from 10:30 a.m. to 3:45 p.m. On this date, there were 37 children present in four classrooms and in the care of seven staff members. There was one additional staff member available to assist if needed. There were 53 children enrolled in the center. Six staff members' records and five children's records were reviewed. All four classrooms were toured and observations were made in two of those classrooms. Six staff members and four children were interviewed. The medication and medication paperwork were reviewed for three children. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Standard #: 22VAC40-665-540-A Description: Based on review of six staff records and verification from the director, one staff member's tuberculosis (TB) screening had been completed prior to 30 calendar days of the date of employment. Evidence: 1. The record for Staff Member #2 contained documentation of having received a TB screening on 11/14/2018. Date of employment for Staff Member #2 was 03/25/2019 more than 30 days after the TB screening. Plan of Correction: The director is now aware TB screenings must not have been completed more than 30 days before the date of employment.
