Comments:
An unannounced renewal inspection was completed on 04/24/2019. There were 25 children, ages 3 years-10 years, in the direct care and supervision of 4 staff members during the inspection; additional support staff were also on-site during the inspection. The inspector reviewed 5 children's records, 5 staff records, 4 applicant records and 1 agent record during the inspection. The staff stated that there is no medication administered. The children were observed in the following activities: nap time, during the transition from the extended day program to the afterschool program, during homework time and having an afternoon snack. The inspector reviewed the parent handbook, the emergency preparedness plans and the emergency supplies during the inspection. The inspector discussed the background check requirements, the licensing process and the requirement to view completed background check requirements with the staff. The inspector arrived for the inspection at 1:40 p.m. and departed at 5:00 p.m. If you have any questions, please contact Becky Forestier at 540-309-2835.

Description: Based on record review and discussion with the staff, the facility failed to ensure that when new persons designated as applicant, licensee, or agents who is or will be involved in the day-to-day operations of the facility or who is or will be alone with, in control of, or supervising one or more of the children, takes office or assumes a role as agent, that those applicants, licensee or agents shall complete the three components of a background check (a sworn statement or affirmation, the search of the central registry and a national fingerprint criminal record check) before the end of 30 days after the change. Evidence: 1. The record for Applicant #2 did not contain the following information: the fingerprint eligibility letter. 2. The record for Applicant #3 did not contain the following information: the fingerprint eligibility letter and the search of the central registry. 3. The record for Applicant #4 did not contain the following information: the fingerprint eligibility letter, the search of the central registry and the sworn statement or affirmation. 4. The record for Agent #1 did not contain the following information: the fingerprint eligibility letter, the search of the central registry and the sworn statement or affirmation. 5. All Applicant and Agent changes took place in July 2018.

Plan of Correction: The applicants and agents will be asked to submit the information.
